On 2/10/19 at 0726, a release of 5 gallons drilling fluid onto the well pad was reported to the TAS by operator representative Angelo Albanese. Spill is reported to have been discovered at 0545, is contained, and has not impacted any waterways.

A follow-up inspection will be conducted.

2841867 Primary Facility 2019-02-11 Administrative/File Review In Compliance with Policy Admin inspection in response to a 5 gallon release of drilling fluid to the well pad previously reported under inspection record 2841865.

Operator representative Derek Zofchak provided additional details surrounding the release to the Department via email. See attached.

2841919 Primary Facility 2019-02-14 Administrative/File Review In Compliance with Policy Admin inspection in response to a 5 gallon release of drilling fluid to the well pad previously reported under inspection record 2841865.

The Department contacted Operator representative Derek Zofchak by phone. Mr. Zofchak confirmed that of the 10 gallons drilling fluids (freshwater and soap) released, 5 gallons spilled onto the well pad and did not leave the well pad. Because the well pad was frozen, clean-up involved scraping the fluids off the pad and into a roll-off box (although a drum would have sufficed due to the minimal amount of contaminated materials). Disposal documents will be submitted to the Department upon disposal.

An on-site follow-up inspection will be conducted.
