2717705 Primary Facility 2018-04-12 Incident- Response to Accident or Event Violation(s) Noted Inspection in response to the Operator report of a spill to the ground. On 4/11/2018, the Operator reported a 54 gallonOn 4/11/2018 at 10:46 AM, A Range supervisor noticed diethylene glycol dripping from the #3 GPU at the Gulick well site. Upon further investigation it was determined that a few bolts on the fire tube gasket connection were not torqued to specification thus leading to the release. The bolts on the #3 GPU were immediately tightened, stopping the release. Absorbent pads were deployed to soak up all standing fluid and the remaining diethylene glycol in the #3 GPU has been vacuumed out in preparation for the unit to be moved. Based upon the initial diethylene fill level versus the level in the #3 GPU when the release was identified, it was determined that approximately 54 gallons of diethylene glycol had released to the graveled well pad. The #3 GPU is scheduled to be moved today 4/12/18 and Weavertown Environmental Group is scheduled to remediate the impacted area.was released to the well pad. A

2724970 Primary Facility 2018-04-30 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection related to a release reported on 4/12/2018. At the time if inspection the Operator has back filled the area impacted by the release. The GPU has also been plumbed for production, however the GPU was not operating. The Operator was actively stimulating the well that will supply the unit. The GPU units on site contain regulated substances and are is not equipped with secondary containment. The Department recommends that the Operator continue to follow the spill policy for remediating large releases. Additionally the the Department recommends that the Operator place all GPU units, containing regulated substances, in secondary containment.
2750119 Primary Facility 2018-07-09 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection related to a release of a regulated substance on 4/11/2018, see inspection report 2727705. At the time of inspection, the GPU unit appeared to be equipped for production, however the GPU was not equipped with secondary containment. According to an email from Operator Rep. Denny Furmanek, received on 7/9/2018, the Operator submitted confirmatory samples and a Site Characterization Report on 6/19/2018. The Department notes that 90 days from the release date is 7/23/2018. The Department plans to follow up with Environment Cleanup regarding the status of the Act 2 review. At this time the Operator is in compliance with the Department most current spill policy, however the GPU unit is not equipped with secondary containment.
