•Landslides/slumps along the access road have not been remediated but have do not seem to have gotten worse. No violation. •A small drilling rig and associated equipment was on site at the time of inspection.

2485987 Primary Facility 2016-01-22 Incident- Response to Accident or Event In Compliance with Policy On 1/22/16, I performed an incident inspection at the Gulick site in response to a spill report received that morning and made the following observations:

•Per the operator a hose disconnected/was not connected properly while transferring reuse water between from a truck to a storage tank allowing roughly 50 gallons to spill out of containment and onto ice on the pad. Of the 50 gallons spilled roughly half was reported to have been immediately removed via vacuum truck before the ice was melted. The remaining amount was to be removed once the pad thawed (pad surface was hard frozen at the time of inspection).

2486296 Primary Facility 2016-04-07 Compliance Evaluation No Violations Noted On 4/7/16, I performed a compliance evaluation inspection at the Gulick site in response to a spill report received 1/22/16 and made the following observations:

•Visual inspection indicated that the spill was fully remediated; vegetation near location of spill was not visibly impacted. •The operator provided receipts from Weavertown Environmental detailing materials removed and disposed. •The containment and storage tanks where the spill occurred had been removed at the time of the inspection.

2514393 Primary Facility 2016-04-19 Routine/Partial Inspection No Violations Noted On 4/19/16, I performed a routine partial inspection at the Gulick site in response to an operator request for review of a minor modification and made the following observations:

•The operator reported that the landowner was requesting access to a vegetated slope below the well pad and needed a pipe and gravel crossing be added to an existing roadside culvert to allow access. This request was approved as a minor modification. •The vegetation cut slope north of the site entrance continues to be sparse with a number of small slumps present. No violation. •Two erosion channels along the access road have been repaired with rock linings. No violation.
