The runoff collection ditch and subsequent sediment basin implemented onsite required maintenance after the most recent rain/storm event. Agreed that the following actions should be taken: 1)The exposed hillsides should be temporarily seeded & mulched, 2) The ditches should be stabilized with rock, 3) The sediment basin should have a portion of the deposited sedimented pulled out so as to insure that the basin continues to function properly, and 4) The sediment basin outlet should be re-rocked to insure it continues to function properly. Mr. Rice stated that these actions will be undertaken as soon as possible.

The well site appeared to be in compliance. A well tag was present. The access road was in good condition. The site was partially restored; the frac pit was still in use to provide service for wells 3H and 4H. There was no evidence of spills or leaks around the tanks and no accelerated erosion was observed at the time of inspection. Some excess material (pallets, bags of cement etc) was being stored on site and should be removed soon. The operator should contiune to inspect the E&S controls routinely and after each runoff event until the site is fully restored
