2453049 Primary Facility 2016-02-23 Complaint Inspection Violation(s) Noted On February 23, 2016, I performed a complaint inspection at the JW Bell 1 in response to a report of oil entering a stream and made the following observations:

•At the time of inspection the site was in poor repair with multiple pieces of derelict and damaged equipment visibly leaking crude oil and oil based sludge to the ground. Specific incidences are noted below. -A spring under the building on site appears to be collecting small amounts of oil and carrying them to the stream. -The larger storage tank appears to have a channel dug or eroded from the valve area leading directly to the stream bank. Blotting the area where the channel enters the stream with oil absorbing cloth indicated substantial oil present. •The smaller storage tank has no functional cover and is roughly 75% full posing a safety hazard to wildlife and people. •At the time of inspection the operator had not reported nor taken corrective action in response to any of the above spills or contamination though much of it is years old. -->S

2494755 Primary Facility 2016-02-24 Follow-up Inspection Outstanding Violations - No Viols Req'd On 2/24/16, I performed a follow up inspection at the JW Bell 1 well in response to a request for a meeting by the operator and made the following observations:

•The owner Raymond D. Langer reported being unsure of how to clean up the site and requested guidance in the matter. Mr. Langer also expressed disagreement with the Department’s request for cleanup of spilled material expressing the site had been in its current state for years and “wasn’t hurting anything”. •The site is noted to have a number of features that, provided the unrestricted access to the site and close proximity to a housing development, pose significant safety risks including damaged and unsecured ladders up the derrick and storage tanks as well as an open topped storage tank full of oily sludge noted in the previous inspection. •The operator reported that he believes any product leaving the site via the stream was being gathered and sequestered in a swamp that he stated to be somewhere farther downstream (it was not located). •Shortly

2495397 Primary Facility 2016-03-31 Follow-up Inspection Outstanding Violations - No Viols Req'd On 3/31/16, I performed a follow up inspection for JW Bell 1 site in response to an NOV sent 2/26/16 in regards in oil spilled on the ground and entering the waters of the commonwealth and made the following observations:

• The operator provided a response to the above mentioned NOV stating that he planned to start remediation work on 3/30/16. • Little to no evidence was witnessed at the time of inspection to indicate that the operator had made any progress in cleaning up the site though most of the product had been removed from the smaller storage tank indicating the site had been serviced.

2495400 Primary Facility 2016-04-07 Follow-up Inspection Outstanding Violations - No Viols Req'd On 4/7/16, I performed a second follow up inspection at the JW Bell 1 site in response to the operator’s statement in his NOV response dated 3/17/16 that cleanup would begin 3/30/2016 and made the following observations:

• Oily material continued to seep in abundance from under the locked building on the site into what appears to be a spring that led directly into the stream that crosses the site. • Soil and leaves contaminated with black and brown oily hydrophobic material was found in multiple locations around the site. • Product from the smaller storage tank on site remained on the ground around the tank in congealed masses. • The site continues to show little to no progress towards remediation.

2495402 Primary Facility 2016-04-22 Follow-up Inspection Outstanding Violations - No Viols Req'd On 4/22/16, I performed a follow up inspection at the JW Bell 1well and made the following observations:

• Soil had been removed along the watercourse leading out from under the building on site and the spring was partially blocked/filtered by a pair of straw bales. A sheen was still observable on the water in the disturbed area and dark contaminated soils still remained directly adjacent. The soil was found piled on a piece of plastic sheeting nearby and covered with a tarp. • A loose berm had been constructed part of the way around the larger of the two storage tanks on site replacing the chute previously leading from the large tank into the stream. This berm did not serve as containment as it still provided a breach into the adjacent disturbed wetland area at the time of inspection. • The pool of water under the tap on the large tank possessed a scummy surface coating suggesting ongoing contamination consistent with the oily product coating many equipment surfaces and dark staining of the soil around th

2495405 Primary Facility 2016-06-02 Follow-up Inspection Outstanding Violations - No Viols Req'd On 6/2/16, I performed a follow up inspection at the JW Bell 1well and made the following observations:

• The remained in effectively the same conditions as the previous inspection with no evidence observed indicating that the further remediation work was performed at the site. • Oily material was still found to be seeping into the spring and disturbed area between the building on site and the stream. Extensive sheen was found on the water in this area of site and blotting with oil absorbing batting picked up brownish orange residue. • Small areas of sheen were found on water pooling throughout the site. • Piles of contaminated soil remained in two separate location on the site.

2495408 Primary Facility 2016-06-03 Follow-up Inspection Outstanding Violations - No Viols Req'd On 6/3/16, I performed a follow up inspection at the JW Bell 1 well accompanied by WQS Supervisor John Carson to discuss efforts to accelerate remediation of the site.
2495599 Primary Facility 2016-06-15 Follow-up Inspection Outstanding Violations - No Viols Req'd On 6/15/16, I performed a follow up inspection at the JW Bell 1 well site accompanied by Environmental Group Manager Dan Counahan and Water Pollution Biologists Michael Stephan & William Brogan and met with the operator to discuss efforts to accelerate remediation of the site. To ensure cleanup of the site is implemented in a timely fashion and to required standards a spill field order was issued to the operator.
2509972 Primary Facility 2016-07-29 Follow-up Inspection Outstanding Violations - No Viols Req'd On 7/29/16, I performed a follow up inspection at the JW Bell 1 well to evaluate the progress made towards satisfying the 6/15/16 Field Order (noted in inspection eFACTS ID 2495599) and made the following observations:

•The site remained in effectively the same conditions as the previous inspections dating back to 4/22/16 (eFACTS ID 2495402) with no evidence observed indicating that further remediation work had been performed at the site. •Oily material was still found to be seeping into the spring and disturbed area between the building on site and the stream. Extensive sheen was found on the water in this area of site and blotting with oil absorbing batting picked up brownish orange residue. •Small areas of sheen were found on water pooling throughout the site. •Piles of contaminated soil remained in two separate location on the site. The larger of the two piles was noted during multiple inspections to be seeping water onto the ground from under the tarp.
