Spoke with Mark Richardson, and Dominion is still investigating the incident. Mark will send me the report once investigation is complete, and will notify me when theyll be replacing tanks, ladders, etc. in the tank battery. As of this date, preliminary restoration is not complete.

The tank battery is used by multiple wells (2); well identification is clearly displayed for all wells utilizing it. The well is a rabbit well, located within a locked metal fence. Well identification is posted at the wellhead as required. No problems were noted at the time of the inspection.

1363619865 2013-03-15 Production Wells No Violations Received phone call from Deputy Chief Claus concerning an oil tank fire in Chippewa Township, Wayne County. Investigation revealed fire to be on the Blaz 1-D lease, owned by Dominion East Ohio Gas. Upon my arrival, fire had been extinguished and there was no evidence of any enviromental impact. Met with Mark Richardson, Dominion Supervisor, and he has made arrangements to have unburnt fluid in both tanks removed immediately. Follow up call with Mark on Monday the 18th revealed they steamed the tanks out on Saturday morning, and are having special investigators come in to try to determine a definitive cause for the fire. Once the internal investigation is finished, all equipment from the tank battery will be removed and new installed. Mark is to let me know when the investigators will be on site, and when the new equipment will be installed.
