0232135826 2009-03-03 Plug / Plug Back No Violations Arrive today to check on gas bubbles and found that the well had been covered over. Contact Bruce with Bishop Well and he will bring out backhoe and uncover well and welding truck to cap well if the gas has decreased.After waiting for equipment Bruce called and said because of the cold Backhoe did not want to remain running. He is to call if he can get it over .Next Bruce called and said he is there on site and that the well will be dug up in a few minutes. He advised that the well has no more bubbles and wanted to see if I needed to be here before he places lid on well. I advised him to weld cap on and cover well . I was on an M.I.T. for OOGC and had to witness it .
1234443064 2009-02-11 Production Wells Violations Noted Found that well has been hit with some type of equipment , most likely one of the very large loaders. They have broken the pump jack in to parts .The polish rod and tubing along with the wellhead have dropped down over one ft. They all have been bent over most likely the same way that the pumping unit was damaged.They should feel very lucky to be alive .The well did not leak at that time but it could begin at any moment. I also have a feeling that the 8 inch casing has a hole in it . Several wells in this area have had that problem and this one has not had production for 10 years or more.Last it is next to the water well field for the city of Wooster. I feel the only reason it did not explode when they hit the well is that the water had already popped through the 8 inch and loaded the well .

1236695617 2009-02-27 Plug / Plug Back No Violations Arrive today to check on cement and find out if it has remained at surface or has fallen. Found cement at surface with a few tiny champain type bubbles. Advise Bishop to let well stay uncapped until Monday and see if bubbles increase or decrease.
1708958494 2009-02-25 Plug / Plug Back No Violations Bishop Well is to try and pull out the 4 inch .They were able to pull up on 4 inch but could not get rip tool down past the break.Call shooters back and shot off 4 inch at 1367 ft. Pull 4 inch out , run back with 2 3/8 tubing to 560 ft and set combined Berea plug and Bottom of surface casing plug. Plug from 560 ft to 225 ft and from 150 ft to -0- ft. Cement remained at surface after job was complete.

1993328975 2009-02-24 Plug / Plug Back No Violations Arrive to find Bishop Well going in hole with 2 3/8 tubing.After checking computer records advise them that 3100 ft should be enough to set bottom plug.Return and set bottom plug thru tubing from 3100 ft to 2900 ft . Pull tubing up for next plug. 4 inch casing has parted at 230 ft and the plan is to get two plugs set before trying to remove 4 inch.Shot holes in 4 inch at 2070 ft and 2015 ft with 30 ft spread.Next set Lime plug thru tubing across shot holes.Set from 2070 to 1720 ft .
