1508021882 2018-10-22 Final Restoration No Violations Performed final restoration inspection on 10/22/2018 at the request of NW Region Supervisor Steve Ochs. No surface expression of the well was noted, nor any associated tanks, production equipment, or lease roads. All vegetation in the area has been re-established as pasture field. Final restoration does not pass however, as all the tubulars pulled from the well are still present at the location. A landowner waiver must be filed with DOGRM Columbus if the tubulars are to remain. No other problems were noted at the time of the inspection.
