0143022138 2016-01-19 Production Wells No Violations Upon inspection, it was observed that the drilling rig has been removed from the location and a 100 bbl production tank has been set within an earthen dike. The well is in production and consists of 7 surface casing, 4.5 production casing, and 2 production tubing. Due to snowy conditions, I was unable to determine whether the preliminary restoration for the location is satisfactory. I will return to the location this spring to evaluate the preliminary restoration. I was accompanied on this inspection by DOGRM Geologist Jake Glascock.

1471251103 2014-06-09 Drill / Deepen / Reopen No Violations Drilling rig is stood up and there is one piece of drill pipe with the drill bit on it. The pit was dug, but had no liner in it. Drilling had not been started due to their air compressor being down. I spoke with Mr. Bobby Anderson over the phone.
1516090382 2014-06-18 Casing String Witnessed No Violations Witnessed the cementing of the production string on this well. 769 ft. of 4.5 in. casing was ran in 868 ft. of 6.25 in. hole. Good circulation was achieved and kept throughout the job. After the plug landed the pipe was tested at 750 psi. and it passed.
1844062473 2014-06-13 Drill / Deepen / Reopen No Violations Witnessed the running and cementing of 300 of 7 surface casing. Good circulation was witnessed and the hole was conditioned with cottonseed hulls as LCM. Cement was run with a 2% mixture of calcium chloride. Good cement returnns were witnessed at the surface. The cement dropped approximately 1 in the first 10 minutes after cementing. Formation Cementing topped off the cement back to surface and it stayed. A pressure test will be performed on Monday, 6/16/2014.
1896479921 2014-08-04 Drill / Deepen / Reopen No Violations Visited this well location to witness well logging and stimulation activities. Timco Well Services was on location to run a gamma/neutron log to find which zones to perforate. A cement bond log was also run to verify the top of cement (TOC). TOC was found to be at 345 below surface. The well was shot from 867 to 845 with 45 lbs of Dyno Nobel RDX/PETN gelled wireline explosives. 15 of wire was left in the well and will be fished out by Mr. Bobby Andersons service rig.
1943150484 2014-10-01 Administrative Inspection No Violations Mr. Bobby Anderson contacted me via phone call at 2:00 pm and informed me that he has scheduled JT Well Surveys to log this well and shoot it at 10:30 am on 10/2/2014. This will be the second attempt at shooting this well to stimulate production.
2008593260 2014-06-16 Drill / Deepen / Reopen No Violations Witnessed pressure testing of the 7 surface casing that was ran and cemented on 6/13/2014. The pressure test commenced at 9:42 a.m. at 350 p.s.i. and ended at 10:12 a.m. at 310 p.s.i. There was a slight leak on the 2 valve on the cement head, which accounted for most of the drop in pressure. The pressure test was considered satisfactory and the O.K. was given to continue deepening.
