0157185155 2017-02-07 Production Wells No Violations I went to the release location to inspect the remediation efforts today. There was a bulldozer at the end of the lease road. When I arrived at the location there was a backhoe sitting below the dike. Nobody was at the location working on the cleanup today. They removed dirt below the dike and removed dirt and grass in the area affected by the scald to the dead evergreen tree. Inspection of the well found fluid to be seeping out from the threads on the tubing head. There was also gas leaking out around the bull plug on the 8.625 inch diameter casing head. I notified Sam Johnson of my findings at the well.
0373932950 2017-01-03 Administrative Inspection No Violations Sam called me back after checking his notes to give me more specific dates. He said the spill happened on the afternoon of June 17th. The water was hauled by them on the 20th to their injection well from the Craig well. This was to verify an earlier statement and to acquire specific dates.

He asked for more time because of the weather and the fact that the land owner leases the ground for hunting. I explained I would have to call him back with a definitive answer.
0785814135 2017-01-04 Administrative Inspection No Violations I tried to call Brian back but only got a fax machine ring. I then called Sam Johnson back and explained that they would need to put their request for an extension in writing with the reasons and the plan of action for completing the work. I also explained that the pit on the vocational well will need closed in and that a notice would be issued. I also told Sam that the Sullivan well was leaking around the bull plug and that it would need to be tightened to prevent the escape of gas.
0903647059 2017-01-03 Administrative Inspection No Violations I called Sam Johnson who said the water was hauled to their WES injection well with their own trucks. He was not able to guess at the amount of brine that was lost outside of the dike. He said he could look and tell me the date the spill was noticed and the brine was hauled from the tank. He also stated he miss spoke when he said the area was seeded and mulched, He corrected the statement and said it fertilized and seeded.
1258197731 2017-02-07 Administrative Inspection No Violations Sam Johnson returned my call. I left a voicemail earlier to inquire about where the contaminated soil was going from this well. Sam called me back with the coordinates and explained that it was near their compressor station straight back the lease road from the intersection of Shay rd. and Twp. Rd 34. He also added that it was near well API # 22913. He stated that it was in a lined pit.
1297511758 2016-12-22 Production Wells No Violations I was accompannied by Supervisor Eric Conrad who was conducting a ride along with me. Inspection of this well brought me to the tank battery first. The identification at the tank battery was not correct or filled out properly. The tank battery was comprised of a 210 barrel steel tank with no plugs in the top of the tank, meter and a separator. The dike drain was open at the end and the cap that was on the end was unscrewed. There was a very significant and large scald area out side the dike. That using my gps I concluded was about 75 feet wide and traveled 250 feet downhill. There were at least 4 cedar trees in the affected area 3 of these trees that were closer to the source were heavily distressed. The production tubing in the well was shut in and disconnected. The 4 1/2 inch diameter pipe was leaking around the top of the tubing head no gas was detected and it was just clear fluid. A field test with Quantabs maxed out the low strips indicating at least 600 ppm chlorides. The 8 inch diameter casing was leaking below the casing head. These two leaks were causing pollution and contamination around the well head. Further investigation will be needed to deduce why the 4 1/2 inch diameter and 8 inch diameter were full of brine causing the casing heads to leak. When we returned to the truck Sam Johnson, Productin Manager for Condevco Washington Operations, pulled up. He addmitted that the tank over ran back in June or July. He said that he tried to call Andy Thomas but the number was wrong. He said that they seeded and mulched the area then. All contaminated soil will be removed and taken to an epa approved landfill. Noncontaminated soil will be brought in to replace the contaminated soil removed. All affected areas will be seeded and mulched to allow vegetaion to reestablish to prevent erosion. The dike drain shall be made to prevent the escape of fluids. The tank battery shall be properly identified with the Company name and telephone as well as the other required inf
1484920617 2016-01-17 Administrative Inspection No Violations On January 13, 2017, I received a written request from Heinrich Enterprises requesting an extension to complete work on the well site. On January 18, 2017, I mailed a written response to Heinrich Enterprises offering them an extension until February 20, 2017 to complete all work on the site.
1599743280 2018-02-12 Production Wells No Violations This well was issued a compliance notice from Inspector Matt Fulk for well operation causing pollution and contamination, well sufficiently equipped to prevent the escape of oil and gas, failure to legibly identify well, dike or pit not able to prevent escape of brine or other wastes, and defective casing, leaking well. The inspection this day revealed that this well is equipped with 8 5/9 inch diameter surface and a 4 1/2 inch diameter production casing. The valves on the wellhead are closed but the well is equipped in a manner for production. The areas mentioned by Inspector Matt Fulk in his reports have been cleaned up and there are no signs of any contamination or spill. The wellhead is equipped and repaired in a way that there is no fluid actively leaking from the well. There is proper identification on the wellhead and the tank battery. The tank battery is equipped with a 210 barrel steel tank and a vertical separator. There is a proper earthen dike built around the tank. There are no signs of the original pollution and contamination, the well is properly identified, and the leak has been repaired on the well. This well is now in compliance.
1683264804 2017-01-03 Administrative Inspection No Violations Called Heinrich Inc, spoke with receptionist and asked to talk to someone who could answer questions concerning the Craig well I issued a compliance notice for. She stated that Brian Chavez wasnt in and would not be in till tomorrow and that this was indeed the best number to reach him at. She then gave me Sam Johnsons cell number. I called Sam Johnson to get some information about when the spill occurred and if brine was hauled from the location.
