0102698472 2014-12-12 Production Wells No Violations On the day of my inspection I received a phone call from Trevor Shaffer with Enervest at 11:20 am that they had a fire and a release of fluid on the pad. The Fire occurred at approximately 8:45 pm on 12/11/2014. A 911 call was placed, but the crew that was on site quickly put the fire out. The fire department was notified that everything was under control, and did not need to finish response to the site. There was no injuries or any damage to any equipment. In Enervest flow back process they are capturing the butane and liquids that were used in the frac. They are captured in a pressurizes vessel. The vessel has a tube that goes up the center of the tank and has pressure regulator on the tube, at the point that an over pressure occurs at gas is then sent to the flare stack. When the gas was sent to the flare stack a slug of fluid went the flare stack also. The fluids caught fire at the flare stack and also had fire on the ground. The area affected was 10 feet wide and 30 feet long. There are no free fluids on the surface and none went off site. All was confined within the pad and around the flare stack.

The discussion held was about safety, the road routes, the curfew on the roads. The projected date of the frac is 11/05/2014, there will be 12 stages and one stage will be done every other day until complete.
0876945807 2014-09-25 Drill / Deepen / Reopen No Violations On the date of inspection I was on site to witness the surface casing string at the Patterson 480 rig. I was informed by company man David Greeson that something was wrong with the bit they were using. Drilling had slowed down and they would be forced to trip out with 50 feet left to drill. I left the site and would return when they were ready to cement.
0944463635 2014-11-13 Completion Testing No Violations On the date of inspection I was on site with ODNR SE Regional Manager Rocky King, ODNR SE District Manager Brent Bear, ODNR Inspectors Pat Shreve and Randy Akins. We witnessed Stage 9 of the well stimulation. The well was stimulated by Gas Frac. Refer to Pat Shreves inspection report and stimulation report for further details.

Gas Frac was unable to complete the stage due to flare equipment issues.

1139770898 2014-09-30 Casing String Witnessed No Violations On the date of inspection I was informed by company man David Greeson that the Patterson 480 rig had drilled a 12.25 inch diameter hole to a depth of 2120 feet. 9.625 inch casing weighing 36.0lbs/ft was set to a depth of 2108 feet. Universal Well Services established circulation and pumped a lead cement of 470sk of Type 1, 15.2ppg, yield of 1.27, 3% Calcium, 0.25lbs/sk of flake. Universal then pumped a tail cement of 215sk of Type 1, 15.6ppg, yield of 1.20, 3% Calcium, and 0.25lbs/sk of flake. Universal displaced with 154bbl of water and 23bbl of cement returned to surface.
1410795376 2014-09-15 Drill / Deepen / Reopen No Violations On the date of inspection I met with Enervest Drilling Manager Gavin Reed. I witnessed that a cellar and conductor were in the process of being drilled by Express Energy Services. The well pad was approximately 90% complete and was constructed by Ellison Dozer. ODNR Engineer Jason Simmerman had previously visited the site and brought up drainage issues to Enervest and will be following up once he receives the latest well pad construction plans. Joe Little Excavating was on site completing electrical operations for the site. ODNR Inspector Randy Akins was on site to discuss the drilling and cementing program. I contacted ODNR Hydrogeologist Rick Trippel to ensure that an up to date Pre-Drill Water Survey had been completed and submitted to ODNR. Rick Trippel will be following up with Enervest concerning the Pre-Drill Water Sampling status. Rick was unable to locate the Pre-Drill Sampling results for the well and Enervest was unable to produce them at the time. After consulting with District Supervisor Brent Bear and Regional Supervisor Rocky King, Randy Akins shut down the auger rig from drilling the conductor until Pre-Drill Water Sampling results could be produced by Enervest.
1411060958 2014-09-18 Casing String Witnessed No Violations On the date of inspection I was informed by Express Energy Services that they had drilled a 26 inch diameter hole to a depth of 100 feet. They then set 20 inch diameter 80lbs/ft of conductor casing to a depth of 100 feet. Nabors Completion & Production Services established circulation then pumped 200sk of Class A cement at 15.6ppg, 1.18 yield. Additives includes 2% CaCl, and 0.25lbs/sk of Celloflake. Nabors pumped 32bbl of fresh water for displacement. 12bbl of cement returned to surface. Duration of inspection also included a pre-spud meeting. ODNR Inspector Randy Akins and SE District Supervisor Brent Bear were present for meeting.
1411745878 2014-09-26 Casing String Witnessed No Violations On the date of inspection I was informed by company man David Greeson that the Patterson 480 rig drilled a 17.5 inch diameter hole to a total depth of 1,008 feet. Wyoming Casing Crew set 13.375 inch diameter casing weighing 54.50lbs/ft to a depth of 990 feet. The Patterson 480 rig pumped water for approximately 3 hours before establishing circulation. Universal Well Services began to rig up on the hole but was delayed due to Enervest Engineer Joe Wade placing an order for additional Lost Circulation Material (LCM) to be delivered. Patterson 480 continued to circulate the hole losing circulation intermittently until the LCM was delivered and Universal rigged up. Universal pumped on the hole to reestablish circulation and then pumped 1,057 sacks of Type 1 cement at 15.6ppg, and a yield of 1.2. 76bbl of cement returned to surface.
1413128134 2014-10-12 Casing String Witnessed No Violations Patterson rig 480 is running 5.5casing of 20lb/ft butress thread to depth of 12,691 feet. There is 51 solid centrilizers on the lateral section. There is 16 bow spring centrilizers on the verticle section. The 9.625casing was ran at depth of 2,000 feet. The drilling rig drilled a 8.75open hole to 7290 feet. They then drilled an 8.5open hole to 12,700 feet. The Two stage tool was set at 6264 feet. Universal is the cementing contractors on site and they will run 505 sacks of 50/50 poz @ 5%gel, .5%CFL-117,.3%CR-3. The rig had landed the string and then the Oil Based Synthetic Mud had to be washed out of hole. Then the 2%KCL Water was used to place around the swell packers and frac sleeves. Then 9.2lb/gal of brine was placed at surface to landing point in the 5.5casing. Universal hooked up and set 2 stage tool at 6264 feet. The circulation that had been established the whole time was lost. The 2000lb of gel was being brought out to run in as two sweeps with 1/4cut celloflake. Circulation was established again and then a fresh water spacer was pumped. Cement was then started. Pumped all 505 sacks. Then washed out lines and displaced down with a wiper plug that closed the ports in the two stage tool. Company will run Bond log and CCL Log.
1415640264 2014-11-06 Completion Testing No Violations I was on location with District and Regional Supervisor, Deputy Chief and floating inspector. There was a safety meeting and the flare stacks would not ignite.
1417011148 2014-11-26 Production Wells No Violations On the day of my inspection I was accompanied by Trevor Schaffer of EnerVest. They are in the process of rigging up the equipment to flow back the well. All the fracking operation are complete. Flow back will start on Monday, 12/01/2014. The tank battery is in the process of being built. In today inspection I have found no violations.
