0194088384 2014-10-02 Production Wells No Violations On the date of inspection I found that the tank battery and earthen dike had not changed since my last inspection. There was still a small amount of liquid present in the dike that was field tested at over 6,000ppm for chlorides. The outside of the dike was moist with visible salt crystals. The 8 foot by 4 foot oil contamination was present. Soil approximately 3 feet outside of the dike was field tested at over 6,000ppm for chlorides. At this time I have not been contacted by Northwood Energy Corp. in regard to the previous Compliance Notice.
0487769444 2014-11-13 Production Wells No Violations On the date of inspection I found that some work had been done to the tank battery. One 210bbl tank had been replaced, new gravel was set underneath the tank, and part of the dike berm had been replaced. It was unclear if work was finished on the tank battery yet. I spoke with Corey McCudden with Northwood Energy by phone and he requested that I follow up with Bill Arnholt. He informed me that Mr. Arnholt handled maintenance issues. He suggested sending Mr. Arnholt an e-mail requesting information. I will follow up with Bill via e-mail regarding any further work to be done at the tank battery.
1114294358 2014-11-13 Production Wells No Violations On the date of inspection I was on site to check the status of the earthen dike and tank battery clean up for the Compliance Notice that was issued 9-18-2014. It is apparent that some work had been done to one of the two 210bbl tanks, beneath the tank, and along the east side of the earthen dike. I was unable to determine if Northwood was still in the process of working on the tank battery. I called the Northwood office and spoke to Corey McCudden who informed me that he does not cover the maintenance of production wells. He referred me to Bill Arnholt and suggested that I e-mail him to get an update. I will follow up with Mr. Arnholt to determine if more work is scheduled to be done.

1410197805 2014-09-08 Production Wells No Violations On the date of inspection I witnessed that two 210bbl tanks were located within the earthen dike. Outside of the dike is a vertical separator. The well head is located approximately 2300 feet to the north west. At the time of my inspection I witnessed that there was a tank leaking into and outside of the dike. The dike was not overflowing but fluid was permeating outside of the dike. Using a Quantab I verified that the liquid in the dike was brine with chlorides over 6000 ppm. I contacted Northwood Energy and spoke to Corey McCudden and he would be sending a crew out to clean up the area and determine where the leak was and repair it. There was approximately one half inch of fluid in the dike in the northeastern corner.
1422558008 2015-01-29 Production Wells No Violations On the date of inspection I am on site to follow up on Compliance Notice 1848770120. I have found that the 210bbl tank on the right as you enter the lease road has been replaced. This was the tank that appeared to be leaking on my previous inspection. Below the new tank there is new gravel where there had been brine before. The earthen dike where there was previously brine has been replaced. There are no elevated chlorides at this time. I find that the well and associated tank battery is in compliance at this time.

1848770120 2014-09-17 Production Wells Violations Noted On the date of inspection I witnessed that two 210bbl tanks were located within the earthen dike. There is also oil staining at the tank access point that measures approximately 8 feet by 4 feet. I had previously contacted Northwood Energy representative Corey McCudden about the contamination issue on 9-8-14. It is required that all brine and contaminated soils within and outside of the dike be removed and hauled away from the location. The area of the dike where brine has migrated out of the dike should be replaced with an impermeable material such as fire clay and compacted. If there is a tank leaking it should be repaired. The oil spill mentioned above that 8 feet by 4 feet located along the access road should be removed and hauled away from the site.
