0094742456 2016-03-07 Production Wells No Violations This is a follow up inspection to the oil spill. The cleanup is continuing. The interceptor ditch is working and the land owner is continuing to pump the fluid out of the ditch into a plastic tank and then draining the fresh water out of the tank, and retaining the oil in the tank. Cleanup around the wellhead need to be done and area to the south of the wellhead needs to have some absorbent pads installed to help with the residual oil that is still on top of the water. Cleanup and monitoring is still needed. I contacted Terry Whitehair with ODNR to update him on the progress and to have him contact Larry White the property owner.
0711712243 2016-02-22 Production Wells No Violations I was accompanied by Josh Shearing with ODNR.

This is a domestic well. This is a follow up inspection to the oil spill. The cleanup is continuing. 1454874907 2016-02-06 Production Wells No Violations Received a phone call from Justin Edwards with the Uhrichsville Fire Department about a possible gas line leak. I Phoned Troy Wells with EnerVest to have them respond to New Port to investigate. It was then determined that it was not EnerVest. The well is well 34-157-2-33736 Owned by Roy White. There is an oil spill on land that cover an area of about 3 feet wide by 100 fet long and different pools 3 feet by 6 feet wide. Approximately 40 to 50 bbl. of oil have been released. I informed Scott King with ODNR and also Dan Goins. The Fire Department is working on the containment. Clean up and further assessment will need to be done in the day light. This a domestic well.
1454953683 2016-03-08 Production Wells No Violations Upon arrivals on location I found Lance Wilcox with Specialized Professional Service, and Larry White the head of the estate for his Father. The crew has placed additional booms in the little tributary to contain the oil from reaching the Stillwater Creek. A vac truck is also on location to assist on the cleanup. Ken Mettler with the Ohio EPA is also on location to access the impact of the spill and to recommend a cleanup plan. Oil Gator is being applied to the surface area to aid in the cleanup. Monitoring will need to be continuingly done. This is a domestic well.

1464018205 2016-05-23 Exempt Well Pre Transfer No Violations On the day of my inspection I found an 8 5/8 X 4 1/2 wellhead with a 4 1/2 X 2 3/8 inch swage on top with a 2 3/8 inch valve on top. The well is currently being used as a domestic well. This well has been a site of a previous oil spill. Larry White has been the contact person. The site needs some cleanup by the wellhead yet. All other areas of the impacted area have been cleaned up.
1552745704 2016-02-08 Production Wells No Violations I was accompanied by Terry Whitehair with ODNR to the well site. Upon arrival on location we found that Mulching has been completed. An interceptor ditch has been install and is working as planned. Approximately 5 bbl. of oil has accumulated in the ditch and is not reaching the water bank. We met with the land owner Larry White to discuss further clean up and the concern about the weather this week. We will need to continue to monitor cleanup until cleanup is complete.
