0705801502 2018-10-31 Production Wells No Violations This inspection was a follow up to a compliance notice number 964217898, issued on 4/7/16, by ODNR Inspector Randy Akins and updated complaint # 3307. Upon arrival on location, I witnessed the well was shut in and no domestic gas was being supplied. The well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, and a pumping unit ran with an electric motor. The overhead electric line had been removed. The production equipment included a steel 100 barrel tank, a concrete vault, a horizonal separator, and a chart recorder within an earthen dike. The production report was up to date but had not showed production since 2010. Proper identification was not present on the tank. Called and talked to the well owner. He states he will reapply for another plug permit and schedule to plug well.
0964217898 2016-04-05 Production Wells Violations Noted On the day of my inspection I found the well to be idle. The valves were in the off position and the belts for the electric motor are missing, and the electric power line has been removed. In checking the production records I found that 2001 was the last year production, and the last of reported production was in the year of 2012. Well needs plugged. The owner/operator shall submit a written plugging schedule to Inspector Randy Akins at 2207 Reiser Ave. New Philadelphia, Ohio, 44663 within 30 day, per OAC 1501:9-11-05(B), and plug the well within 180 days. The owner/operator will be notified if the plugging schedule is approved or denied.

An NOV was issued on 4/7/2016 with a remedy date of 10/6/2016, and to this date nothing has been done. There has been no contact from Killbuck Creek Oil Company.
