0048859547 2014-09-18 Production Wells No Violations We began our investigation at the location of a 30bbl tank and sales line along Atkinson Creek Rd. SE, Perry Township, Tuscarawas County, Ohio. The purpose of the investigation was to respond to a complaint received from Mr. Mike Pivonka which referenced his property and that of his brother-in-law Mr. Clyde Fuller. Mr. Pivonka had claimed to smell gas on his property, particularly H2S. Our investigation included determining whether H2S or any other gases were present near the locations of Mr. Clyde Fullers properties or that of his brother-in-law Mr. Mike Pivonka. A complaint was taken from Mr. Fuller in the past referring to the odor of H2S on his property and the case was closed due to a lack of evidence. Personnel on site were ODNR Inspector Randy Akins, SE District Supervisor Brent Bear, and ODNR Inspector Brandon Trigg. Our beginning coordinates were 40.22027 N/-81.33530 W. We used Honeywell PhD6 gas handheld gas monitoring devices around the tank, sales line, and motor valve. We did not detect H2S around the equipment. We checked any pipeline road crossing vents for gas throughout our route.

We monitored around the home and property adjacent to the home as well as the exhaust unit that came out from the house on the west side and where a gas line had previously been connected near the same location. We did not detect H2S around the home, adjacent property, or along the roadway from our starting point. -81.342274. We monitored gas levels around the perimeter of his rental home, barn, and the low lying areas near the stream to the south of the road. We did not detect any gas at this location. We continued west to the location of the R. Fuller #1 well, separator, and tan

The valves are in the open position. The tank battery is made up of one 100 bbl. tank, one 50 bbl. tank, and a horizontal separator, and a recording gauge, in and earthen dike. The production reporting is up to date, the tank is properly identified, the production reporting is up to date, and there are no violations at this time. There are no indications of any H2S at this time.

1406446600 2014-02-27 Production Wells No Violations I arrived on location with Oil and Gas well inspector Mark Moreland, to conduct an inspection of oil and gas wells located within 1/2 mile radius of H2S complaint no. 3701. Inspector Moreland monitored the well site with a calibrated Sperian PHD6 gas meter for this inspection and showed no detection of H2S.
1420465978 2014-01-05 Production Wells No Violations On the day of my inspection I found a 4 X 1.5 wellhead and the system was operated by a plunger lift. A horizontal separator, a 50 bbl, and a 100 bbl tank, all in an earthen dike. The tank is properly identified. The production reports are up to date. I had a report of some gas leakage at this location. I called Tipka and they responded to the location and found no gas leakage. On Sunday Freeport fire department was call to the location for an odor of gas. I talked with Gary with the fire department and he stated there was no gas odor. I have not found any gas leakage at this time. I met with the pumper with Tipka and Vito Amian at the location, again no gas leakage was found. The well and the separator have been temporally shut in.

2137425745 2016-08-03 Production Wells No Violations I arrived on site today after encountering a concerned citizen who I met conducting an inspection on another well bore. The person wished to stay anonymous and informed me that he had noticed some odd orders coming from the well bore well he was hunting last year. He asked if I would just perform an inspection on the well for him. When I arrived on location I witnessed the permitted well bore is utilizing a plunger-lift system for means of production. The casing program for the well consists of 8.625 inch diameter surface casing, 4.5 inch diameter production casing and 1.5 inch diameter production tubing. The plunger is being operated by a motor valve and a control box. All valves were in the open position. Production equipment consists of a horizontal separator, a 2 inch diameter meter run, a 50 barrel brine tank and a 100 barrel production tank. All production equipment is placed inside an earthen dike that measures approximately 20 feet by 25 feet at an average height of 1.5 foot. RBDMS records indicate that the well is in producing order and no issues could be noted at the time of my inspection. I did not noticed any odors or detect any gases with my meters. I stated to him I would perform a follow up inspection on the well.
