0737028129 2017-10-26 Production Wells No Violations On the day of my inspection I found an 8 5/8 inch diameter casing, a 4 1/2 inch diameter casing, a 2 3/8 inch diameter tubing operated by a plunger lift system. The production equipment is made up of one 100 bbl. steel tank, a horizontal separator. The tank is properly identified, and the production reporting is up to date.
0742311538 2018-12-12 Final Restoration No Violations This inspection was to do a Final Restoration on this location. Upon my arrival on location, I witnessed that the location had been reclaimed and seeded. The production equipment had been removed and the earthen dike area had been reclaimed and seeded, as well. The lease road had been graded, but not removed. I contacted well owner about a land owner waiver. He stated that he was having trouble getting in contact with the land owner. Once, he contacts the land owner he will have them sign a land owner wavier.
1226595059 2018-07-17 Plug / Plug Back No Violations Upon arrival on location, I witnessed Hagans Well Service crew pulling up on the 2.375 inch diameter tubing. The tubing was set on a packer at 7000 feet. They got the packer unseated, but the tubing would only pull up the well about 30 feet. They decided to go ahead and set the bottom cement plug thru the tubing. The bottom plug will be set from 7721 feet to 7342 feet. Petroset cleared tubing with 30 barrels of fresh water, then pumped a 40 sack, Class A 2% gel cement plug and displaced with 28 barrels of fresh water. Appalachian Well Surveys rigged up to shoot the tubing off above the tubing packer. They shot the tubing off at 7096 feet. The rig crew started to pull tubing out of well to the depth of 5266 feet. Appalachian Well Surveys ran back down in the well to perf the 4.5 inch casing at the Clinton zone. Petroset pumped 120 barrels of fresh water to gain circulation. They gained circulation and started to pump a 65 sack, Class A cement plug and displaced with barrels of fresh water. Rig crew pulled 10 joints of tubing out of well and Petroset reversed circulated back out the tubing.

1531922091 2018-07-18 Plug / Plug Back No Violations Upon arrival on location the second day, I witness the rig crew running tubing back in the well, to tag the top of the clinton cement plug. They ran 10.7 joints of tubing back in the well. They tagged the top of the cement plug at 5018 feet. The rig crew started to pull the remaining tubing out of the well.
1532026107 2018-07-19 Plug / Plug Back No Violations Upon arrival on location on the third day, I witnessed the rig crew had removed all the 2.375 inch diameter tubing out of the well. Appalachian was rigging up to shoot the 4.5 inch casing off at 3771 feet. Rig crew pulled 2 joints of 4.5 inch casing. Petroset established circulation on the well, and then pumped an 75 sack, Class A cement plug. They displaced with 53 barrels of fresh water to the depth of 3420 feet. Rig crew pull 56 joints of casing out of well. Cement crew established circulation in the well and then pumped a 90 sack, Class A cement plug. They displaced with 19 barrels of fresh water to the depth of 1231 feet. Rig crew pulled 29 joints of casing out of the well. Cement crew established circulation, and then pumped a 45 sack, Class A cement plug, until cement returns were seen at ground level. The rig crew then pulled the remaining casing out of the well.
1532089480 2018-07-20 Plug / Plug Back No Violations Upon my arrival on location, I witnessed Cox Well Service cutting the 8.625 inch diameter casing off 30 inches below ground level. The surface plug had settled about 8 feet below surface. The roustabout crew is going to put several bags of Quikcrete to top off well. They had the identification plate ready to be installed on the top of the well. After, they install the plate on the well, they are back filling the small circulation pit and the hole around the 8.625 inch casing. The circulation pit had been sucked out of all free-standing fluid. The production equipment had already been removed from location, prior to the plugging of the well. They will then seed and mulch the location.
