0425511164 2017-12-15 Plug / Plug Back No Violations Nicks Well Plugging tripped 114 joints of 1.5 inch diameter tubing down to 3727 feet. A 300 foot big lime plug was set from 3427 feet to 3727 feet within the 7.875 inch diameter borehole. The plug consisted of 86 sacks of class A cement weighing 15.6 pounds per gallon with a yield of 1.18 cubic foot per sack. The plug was displaced with 8.5 barrels of fresh water. 75 joints of tubing were pulled to 1258 feet within the 7.875 inch diameter borehole, 100 feet below the 8.625 inch diameter surface casing. A 200 foot plug was pumped 100 foot within the 8.625 inch diameter surface casing and 100 foot within the 7.875 inch diameter borehole. The plug consisted of 61 sacks of class A cement weighing 15.6 pounds per gallon with a yield of 1.18 cubic foot per sack. The plug was displaced with 2.7 barrels of fresh water. The remaining 38 joints of 1.5 inch tubing were tripped out of hole.

0618773581 2017-12-18 Plug / Plug Back No Violations Nicks Well Plugging tagged the top of the bottom of surface plug at 1060 feet. The plug remained at its required depth. Six joints of 1.5 inch diameter tubing were tripped into 200 feet below surface grade. A 200 foot plug was set to surface. The plug consisted of class A cement weighing 15.6 pounds per gallon with a yield of 1.18 cubic foot per sack. The remaining 6 joints of tubing were tripped out of the hole and the cement top was 4 foot from surface grade. The hole was topped off and the cement stayed at surface. A total of 81 sacks of cement were pumped for the surface plug.
1513171844 2017-12-12 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging operations. This well was drilled to the Clinton Sands formation and was completed through 4.5 inch diameter casing with 21 perforations from 5135 feet to 5270 feet. The well was equipped with 8.625 inch diameter surface casing ran to 1158 feet, 4.5 inch diameter production casing ran to 5312, 1.5 inch diameter tubing, and a plunger lift system. 60 parts per million of Hydrogen Sulfide were detected at the wellhead. Appalachian Well Surveys, Nicks Well Plugging, Haddad and Brooks Incorporated, and Safety First were on location. Safety First set up a hydrogen sulfide monitoring system around the well location. Nicks Well Plugging nippled up to the 1.5 inch diameter tubing and attempted to pump through the tubing. The pump pressured up to 1000 pounds but could not pump through, confirming that there was a plunger still within the 1.5 inch diameter tubing. Appalachian Well Surveys rigged up and tripped in hole with a tubing cutter. The 1.5 inch diameter tubing was shot free at 5222 feet in depth. Appalachian Well Surveys tripped out of hole, rigged down, and exited the location. Nicks Well Plugging pumped a 400 foot plug through the 1.5 inch tubing a crossed the perforated portion of the 4.5 inch production casing from 5222 feet to 4822 feet in depth. The plug consisted of class A cement weighing 15.6 pound per gallon with a yield of 1.18. 12 barrels of fresh water were pumped for displacement.

2033634503 2017-12-13 Plug / Plug Back No Violations Nicks Well Plugging blew down the 8.625 inch diameter surface casing and discovered high Hydrogen Sulfide readings up to 300 parts per million. The Safety First alarms went off and all personnel temporarily evacuated the wellsite until the gas pressure decreased and the hydrogen sulfide readings decreased. B and E Well Treating arrived on location with Sweetener to help kill the Hydrogen Sulfide Gas flow. 55 gallon of sweetener was mixed with 150 gallon of water and pumped down the well and the Hydrogen Sulfide gas flow was stopped. Nicks Well Plugging tipped within the 4.5 inch diameter casing with their ripping tool and ripped the 4.5 inch casing at 3735 feet, 248 feet below the top of the big lime. Nicks Well Plugging pulled up on the 4.5 inch casing and the block line began to break. The 4.5 inch casing was sat down and secured within the slips. Nicks Well Plugging made the necessary repairs to the rig.
