The gravel driveway used to access the site has been finish graded. The access from the gravel drive to the well and the wellsite has also been finish graded and seeded and vegetative growth established. The visible piping, downgradient of the well has been removed.

I obtained a signed Right-of-Entry (ROE) from the landowner (Michael and Sarah Cady) at address 7318. It will be their driveway which the contactor will be using to access the well.
I walked the perimeter of the location with my Sperian gas meter to check levels of H2S and natural gas - none detected.

- none detected. 

The 	Contractor excavated around the well bore, to a depth of approximately 4, and found 8.625 diameter casing with a well head and 4.5 diameter casing rusted off at top of slips.  The contractor had to replace the side port fittings on the wellhead, due to corrosion.
The 	Contractor welded an 11 diameter collar onto the wellhead to install a control head and then positioned the rig over wellhead and commenced the clean out.
0068119658 2014-03-27 Orphan No Violations No site inspection was conducted on 03-27 due to a meeting held in the New Philadelphia office with Gene Chini (Orphan Program Manager), Keith and Jeff Moore and I to discuss what remedial action that will be taken to correct the top cement plug, which is leaking. The decision was made to drill out the top plug to a depth of approximately 100 and re-plug. Moore will submit a cost estimate for this remedial action.

This follow up inspection was conducted on 04-07 for contract compliance. - The landowner visited the site and inquired on the drilling status. I advised him that the surface plug had to be drilled out and re-set. -5 minutes to see if the cuttings would drop out and then re-start the mud pump. When Keith called back he stated that he could tell the cuttings had dropped out due the increased pressure on mud pump. I then advised him to mud up the freshwater. This loss of circulation maybe due to a hole or part in the surface casing.

Checked well and found gas bubbling through the fluid in the well bore. Gas readings are as follows: CH4 - .01 PPM, 1% LEL, 4% CO and H2S - 0 PPM.

I walked the perimeter of the location with my Sperian gas meter to check for H2S and natural gas - none detected.

The Contractor was on site and checked the well for pressure. They then ran 2.375 diameter steel pipe down over hill to the access road, where the cement equipment will be set up.
	Cementers arrived, rigged up and connected up to the tubing and pumped to see if the tubing is open.  They mixed 40 sacks of 50/50 Poz mix at 15.1#/gallon and displaced it with 19.5 barrels of fluid to set the bottom plug (5337-4837).

The Contractor called Logging Company and arranged for them to be on location at 1:00 PM.

	The Contractor pulled stretch and rotated the tubing and, after several attempts, the tubing came free. Pulled the tubing out of the hole and shut down for the day.

1345926622 2014-03-19 Plug / Plug Back No Violations Accompanies Inspector Norburt Lowder while witnessing plugging operations on this well. We witnessed the bottom hole plug being pumped. After it was pumped, the tubing got hung in the hole and the wire line company came out and shot the tubing. I also participated with Inspectors Justin Smith, LJ Parkins and Josh Garland in taking an H2S awareness class put on by OBrian Safety Services. They were also there monitoring the site for H2S gas coming out of the well bore.
1395259000 2014-03-19 Plug / Plug Back No Violations Accompanied Inspectors Josh Garland, Andy Thomas, and Justin Smith to this idle/orphan well site to attend an H2S safety training meeting with OBrien Safety Services. After the training, we witnessed the attempted removal of the 2.5 tubing, which had become hung up inside the 4.5 casing. The tubing had been shot by Appalachian Well Surveys in an attempt to free it, but the attempt was not successful.

The Contactor opened the well bore and checked it with their meter and found: .01% - CH4, 2% LEL, 5% CO and 0 PPM - H2S. A meeting is scheduled in New Philadelphia to discuss how to remediate this leakage from the surface plug. The Contractor removed the tank from the tank battery area and then leveled the dike and removed the flow lines. Photos were taken though out the removal.

2038663627 2012-08-29 Orphan No Violations I&O well leaking h2s gas . Complaint from Columbus ( Terracina Maxwell ) concerning a well which is leaking h2s. After checking well I found that thus well has been in the I&O file in Columbus for five years. Waiting for Columbus to plug this well.

-300) to the drilling fluid. There was no fluid loss and the circulated cutting size was .25 to .5 in diameter.

Without warning, the circulating fluid changed to a black foam and they lost approximately 10 barrels of fluid. I advised the contractor to stop circulating and remove all the drilling fluid from half-tank. This black fluid could be coal water coming from a possible hole or part in the casing. I advised the Contractor to bail the fluid out of the well bore so a down hole camera can be run.
