1521141093 2018-03-15 Non Well No Violations Upon arrival on location We met with surveyor Tom Smith, to determine if 2 inch diameter pipe that was sticking up was from a pipeline or was well related. At this point it is not identified. It was decided that we will have it dug out to see if it will expose any well casing.
1523633341 2018-04-13 Orphan No Violations On the day of my inspection, I met with Dan Guspyt and Brett Guspyt. They brought their excavator to expose 2 inch diameter pipe. The pipe had a 2 inch brass gate valve on the top of it. The valve was in the closed position and would not open. We then exposed pipe by digging around the entire pipe. We dug to the depth of 7 feet exposing pipe the entire depth. The soil was sandy and at the bottom of hole we started running into what looked like a clay layer. The pipe was free from any leakage of natural gas or crude oil. After talking with my supervisor Mike Moreland about the situation. We decided to do more investigation on the matter. I told Brett Guspyt to fill the hole and we would be back in touch with them. As we were discussing the situation, the land owner ( Kendra Long ) arrived on site. I asked her about the 2 inch pipe. She does remember the pipe sticking up out of ground, but she could not remember anyone coming around checking it or working on it. As they were finishing filling the hole, I pulled up a map up of all wells in Tuscarawas County. I found a well close to the spot we were working. I went to the RBDMS program and typed in permit number in. I did find plug back report in file. This could be a plug back Berea well. I will now turn over to Orphan Well Program.
1543600992 2018-11-30 Orphan No Violations This inspection was requested by Orphan Well Program Manager, Gene Chini. He needed several pictures of this orphan well to update the well file. Upon my arrival on location, I witnessed the well was equipped with 2 inch diameter tubing and a 2 inch valve. The well was shut in and the location was free of any types of contamination. The production equipment had already been removed from location.
