0007808571 2014-06-18 Production Wells No Violations I was on location for a status check inspection. I found during my inspection the well in production. The tank battery consisted of five, 400 barrel tanks and a 500 barrel tank. A lined steel dike surrounded the tanks. The load lines had plugs and locks. The entire well pad was fenced with a locked gate. An identification sign was posted on the fence at the entrance to the pad. The lease road was in excellent condition. Drainage and erosions controls were in place and working properly. Vegetation was established on the berms.
0340387707 2013-07-11 Drill / Deepen / Reopen No Violations My inspection found an Auger Rig on location drilling the hole for the cellar. BP engineers were on location making improvements to the drainage control surrounding the well pad and along the lease road by adding additional rip-rap rock to the ditches. Grass was well established on the topsoil stockpiles surrounding the pad.
0392051224 2013-11-26 Preliminary Restoration No Violations My inspection found all of the production equipment installed. A steel dike with a liner and gravel surrounded the production tanks. The tank battery consisted of five, 410 barrel tanks and a 500 barrel tank. The water and oil load lines were locked. There was a locked gate at the entrance to the well pad. Drainage and erosion controls were in place and working properly. Vegetation was established. The lease road was in good, stable condition. The proper identification was posted.
0844691345 2013-07-20 Drill / Deepen / Reopen No Violations I was on location for the cementing of the 9 5/8 casing. The drill depth was 2527 ft, with a 12.25 inch hole and 9 5/8 inch intermediate casing placed at a depth of 2527 ft. The proper circulation was established and cementing was started by Halliburton. There was 190 BBLs of class A cement with 2% calcium run for the job. The beginning mudscale of cement was a density of 15.6 pounds per gallon (ppg). The amount of returns were determined to be 38 BBLs. The final mudscale of the returns was 15.1 ppg. The plug was landed and 500 psi was held for five minutes, then reduced to 250 psi and held for another five minutes. After releasing pressure we had 3 BBLs of flow back. The well site was in good repair with the proper containment in place. The lease road and well pad were in stable condition. The permit was in the company mans trailer. Fire extinguishers were readily available during the job.
0908203494 2016-08-12 Production Wells No Violations At the time of my inspection, I found the well not in production. The gate valves on the well are closed. The well is equipped with a christmas tree valve assembly. The access road is in good repair. The main gate was secure with a lock. The well pad is surrounded by a wooden post and wire fence with a secure gate in the northeast corner. Identification signs are posted at the access road entrance and tank battery. There is a steel dike around the storage tanks.

The equipment on the pad consists of the following: 6 Storage tanks Five 400 bbl One 500 bbl 1 Gas processing unit (GPU) 1 Vapor recovery unit (VRU) 1 Production well on the pad Site fenced and man doors locked

1209432791 2014-11-04 Production Wells No Violations I was on location to check for the proper identification sign. During my inspection, I found the BP identification sign had been removed and replaced with Halcon Operating.
1311195361 2017-10-19 Production Wells No Violations I found the well in production. The production equipment are in good repair. There is a steel dike around the storage tanks. Identification signs are posted at the tank battery and access road entrance. The well pad is secure with a locked gate. There are no violations at this time.
1373999041 2013-07-13 Drill / Deepen / Reopen No Violations I witnessed Express Energy Service cement the conductor casing for BP. Express Energy Services operator drilled a 26 Hole #1 to 82 and set 20 casing to 80. The casing had a weight of 94 lb./ft. and a grading of H40. Express Energy established circulation with 11 bbls of fresh water. The operator pumped 160 sacks of Class A cement with 2% CaCl and 1/2 lb. Celflake. The cement was displaced with 24.8 bbls of fresh water to 70. I witnessed approximately 6 bbls of good cement returned to surface. The fresh water for mixing had a pH of 7 and 76 degrees.
1374589196 2013-07-22 Drill / Deepen / Reopen No Violations I witnessed Halliburton cemented the surface casing for BP. Nabors M07 drilled a 17.5 Hole #2 to 344 RKB (Kelly) and set 13.375 casing to 329 RKB. Nabors M07s Kelly was 22 above ground level. The surface casing had a grading of J-55, 61 lbs, and an average length of 42. Halliburton establish circulation with 5 bbls of fresh water. The operator pumped 50 bbls of fresh water spacing followed by 170 sacks of Bondcem System cement. Halliburton dropped the plug on the fly and displaced the cement to 286. I witnessed approximately 17 bbls of good cement returned to surface. Halliburton bumped the plug at 500 psi over operating pressure. After 5 minutes, pressure was released to 150 psi and plug held static. Three cement samples were taken during the mixing cycle at the beginning, middle, and end; all three met the density requirements.
1375799006 2013-07-29 Drill / Deepen / Reopen No Violations My inspection found Nabors M07 drilling at a current depth of 3,270. Identification was posted in the company mans trailer; no violations at this time.
1426800238 2013-09-09 Drill / Deepen / Reopen No Violations I received a call from Koreen Burrows, BP Field Environmental Advisor. Koreen stated that all of the grass on the berms surrounding the well pad was killed by a company contracted to kill weeds at all of BPs well locations. Koreen stated the weed killer lasts up to 6 to 8 months. BP plans to reseed the berms by the Spring of 2014. My inspection found all of the grass on the berms dead. The soil in the berms appeared stable. I noticed new stone lined in the drainage ditches. I took photos of the berms.
1456323803 2016-02-24 Production Wells No Violations I was on location for a status check. My inspection found the access road is in good repair. Identification signs are posted at the access road entrance. The pad is fenced and all man doors are unlocked. The #1H is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the storage tanks.

I called and spoke with Zane Oshurac from Halcon. I instructed Mr. Oshurac to secure all man doors with locks and post an identification sign at the well.

1462949865 2013-08-09 Drill / Deepen / Reopen No Violations My inspection found BP and Nabors M07 rigging down to move to the next drilling location; no violations at this time.
1469001780 2013-07-08 Not Drilled No Violations My inspection found the well pad construction complete. Drainage controls were in place with two sediment ponds located in the northwest and southwest corners. Vegetation was established on the topsoil berms surrounding the pad. Soil stabilization was used on the well pad.
1516131613 2018-01-04 Production Wells No Violations I conducted an inspection on this well with DOGRM inspector, John Fleming. The well valves were open and production records for this well were current. The production equipment was in good repair. There is a steel dike around the storage tanks. Identification signs are posted at the tank battery and access road entrance. The well pad is secure with a locked gate. No violations observed.
1550846383 2013-08-01 Drill / Deepen / Reopen No Violations My inspection found sediment had left the drilling site and deposited in the ditch along the entrance of the lease road. The area of concern was the first 30 to 40. I contacted Koreen Burrows, BP Environmentalist. I instructed her to place rip-rap rock to help prevent further sedimentation and erosion.
1772191171 2013-07-12 Drill / Deepen / Reopen No Violations My inspection found the auger rig drilling a 24 Hole #1. The current depth was 30. Trumbull Construction was adding crushed limestone to the well pad and rip-rap to the drainage ditches; no violations at this time.
2083810928 2013-07-25 Drill / Deepen / Reopen No Violations My inspection found Nabors M07 drilling at a current depth of 1,907. The drilling permit was posted in BPs company trailer; no violations at this time.
