0368381454 2017-05-24 Production Wells No Violations I was on location to assess compliance notice #1462110496 and follow--up line on the 100 bbl. storage tank. The oil impacted soil and vegetation in the tank battery has not been cleaned up. According to Mr. Momosis, he has isolated the 100 bbl. storage tank and plans to replace it with a new tank. The 50 bbl. storage tank will be used as the temporary production tank.

Vortex still needs to: Remove and dispose of all impacted soil and vegetation inside of the tank battery. Repair surface area affected by the oil release. Post the proper identification at the tank battery. Repair the gas leak at the well. Till, grade, and seed the disturbed area from repairing the sales line along Thompson Sharpsville Road.

Photos were taken of the disturbed area from the repair.

The landowner, Lewis Kostoff, call me on Monday, 6/12/17 about Vortex landscaping the area of disturbance. There is clumped soil and tire ruts. I spoke with Charlie Moisio, contracted well tender for Vortex Energy. I told Mr. Moisio the area needed graded and seeded. Mr. Moisio stated he would try to address the issue by next Thursday, 6/22/17. Follow-up inspections will continue until the issue is corrected. I called and left a message with Mr. Kostoff for a return call.

1350657541 2012-10-19 Production Wells No Violations I conducted a production inspection status check. My inspection found the well equipped with a plunger lift unit that was covered by overgrown weeds and thorns. The tank battery was overgrown with weeds and contained one, 100 barrel oil storage tank, one, 50 barrel brine tank, a separator, and no identification. The production equipment had the correct spacing. I noticed no visible spills or leaks. The lease road was good and was shared by permit number 2-2408, but grass covered. I researched our database and found no production reported to Columbus for 2011. I emailed the compliance violation to Jamie Campbell of Big Sky. I will re-inspect the well by 11/19/2012.
