0608141585 2013-05-28 Production Wells No Violations My inspection found the earthen dam working properly. A steady stream was flowing through the dam. The absorbent socks upstream were working properly. There was one absorbent sock downstream from the dam and secured with metal rods. I noticed no visible oil or sheen. The locked connex box and roll-off were still on location. A gate was placed at the entrance of the lease road with a posted USEPA superfund keep out sign. The tank battery location was graded and seeded.
0662163390 2013-05-08 Production Wells No Violations I researched the Trumbull County Recorders website site and found a lease agreement filed between Mr. Percy Eugene Tenney and Parrot Energy on 3/22/2012. I gave this information to the Federal EPA.
1014239823 2013-05-13 Production Wells No Violations I found the Federal EPA removing impacted soil from the site and still cleaning the creek. The fire chief, Federal EPA, and Coast Guard were drawing up plans to perform a controlled burn on 5/14/2013. New gravel was spread onto the lease road.

Currently her son, Gene Tenney, has ownership of the property where the well exists. Mr. Tenney was contracting Parrot Energy (330-637-0151) to service the well. Parrot Energy subcontracted Jim Adams to service the well twice a month. I contacted Parrot Energy about the spill and they notified Jim. Jim arrived on location and said he has not been on location since late November to December. He stated that the well was shut-in and both load lines had bull plugs. Jim contacted Holton, Inc. to dispatched a excavator and dump truck to clean-up all of the impacted soil and gravel to be disposed of at a land fill. 1368109797 2013-05-07 Production Wells No Violations I found clean-up operations continuing with the Federal EPA. Parrot Energy owner stopped by the site to sign a chain of custody form from Wolford Roll-offs to have the impacted soil tested by Cardinal Labs. While the soil is being tested, Wolford will remove the two roll-offs and store them at their facility. Once the tests are complete, the soil will be disposed of at the Republic Services Landfill in Poland, OH.
1369915161 2013-05-16 Production Wells No Violations I witnessed the Federal EPA, U.S. Coast Guard, and the Bristol Township and Mecca Township Fire Departments perform a controlled burn on two sections of the creek. Once the burns were completed, the remaining grass was brush hogged for further clean-up operations. An earthen dam with 6 inch black corrugated drainage pipe was constructed in the creek at approximately 0.75 miles from the production tank. Three rows of 6 inch absorbent socks were placed upstream to contain any additional oil.
1431615808 2015-05-13 Exempt Well Pre Transfer No Violations I was on location for an exempt well pre-transfer inspection. During my inspection, I found the well not in commercial production. The well is equipped for direct flow with the wells 4.5 inch casing reduced with a swage to a two inch flow line. A two inch ball valve on the flow line is in the closed position and secured with a lock. There is no production equipment associated with the well. The lease road is in stable condition with a locked gate at the entrance. I observed the well is over 200 feet from all public and inhabited private dwellings. I researched the Trumbull County Auditors web-site and found that Percey Eugene Tenney owns the property where the well is located. I called and spoke with Mr. Tenney concerning the well. Mr. Tenney stated the well resides on 17 acres. Also, Mr. Tenney stated the well will supply domestic gas to his new home residence, which will be built within the year.

The absorbent socks upstream were secured with metal rods and working properly. I did not notice any oil or oil sheen at the socks or dam. The Federal EPA was packing up equipment. A locked connex box and roll-off container were on location.
1726761977 2013-06-03 Production Wells No Violations My inspection found the earthen dam operating properly with a clean steady flow of water. The absorbent socks upstream and downstream were operating properly. I noticed no visible sheen or oil. The wellhead remained shut-in with a closed ball valve above the 4.5 x 2 swage and a closed ball valve with a lock on the flow line. The roll-off and locked connex box were still on location. A gate was in place at the entrance of the lease road with an USEPA sign.
