1368034197 2013-05-08 Plug / Plug Back No Violations I witnessed Nicks Well Plugging plug the Hughes well for Gas Acquisition. They pulled 4000 of rods with a average length of 25. Nicks Well Plugging broke circulation and pumped 25 sacks of Class A cement through the 2.375 tubing for Plug #1. They displaced with 15 barrels of brine water. They pulled up on the 4.5 production casing and found the string weight indicated much less than expected. I decided to have Nicks Well Plugging remove the 4.5 casing and cement through the 2.375 tubing to ensure proper placement of the remaining plugs. A total of 66 joints of 4.5 casing at an average length of 42 were removed from the well indicating it was separated at 2,766. Nick broke circulation and pumped 54 sacks of Class A cement for Plug #2. They displaced with 9 barrels of fresh water. The pulled tubing to 100 below the surface casing. Nick broke circulation and pumped 99 sacks of Class A cement for Plug #3. I witnessed a good show of returned cement to surface. A total of 4,037 of 2.375 tubing was removed from the well with a average length of 32.
1378384469 2013-09-05 Final Restoration No Violations My inspection found all the production equipment and debris removed. The well site and lease road were graded. Vegetation was well established. The pit was filled, closed, and graded. I received the landowner waiver for the lease road on 5/24/2013.
1946816976 2016-11-30 Administrative Inspection No Violations I contacted the office of Gas Acquisition and spoke with Scott Wilkes concerning the removal of all underground pipelines from the property. The underground pipelines were not on the Form 5, which was submitted on 5/11/2013. Contact information for the landowner, Gary McIntosh, was given to the company. I called and left a message with Mr. McIntosh for a return call.
