0016934276 2016-06-02 Production Wells No Violations Follow-up to Compliance Notice #1285508938 issued on 5/3/2016 due to pollution and contamination issues found at this tank battery and requiring Big Sky Energy Incorporated to post the required ID at this location. My inspection found the flow line from the wellhead to the separator closed and the domestic tap at the wellhead open. The production chart indicated pressure at the separator and the production chart has been changed since the last inspection and has overprinted 3-4 weeks of production. Although the flow line valve between the wellhead and separator indicated a closed position, I could clearly hear gas moving through the separator. The valves on the separator were open at this time. An inspection of the tank battery found that none of the remedial activities required by the prior Compliance Notice have been undertaken to address the pollution and contamination at the tank battery and the proper ID signage has not been posted. Big Sky Energy Incorporated continues to be in violation of Chapter 1501.9-1-07 and 1501.9-9-05(A)10 of the Ohio Administrative Code and Chapter 1509.22(A) of the Ohio Revised Code.

0628708897 2014-03-10 Production Wells No Violations I was on location to perform a follow-up inspection to compliance notice #1393509014, which was issued in reference to Chiefs Order 2014-43. My inspection found a production chart inside the separator with no date. The flow line valves leading to and exiting the separator were open. A valve on the flow line at the well was open. The digital meter on the dump valve at the well was showing a digital number. The well was in production which was in non-compliance of Chiefs Order 2014-43. The well needs to be shut-in immediately.
0773619980 2017-09-01 Production Wells No Violations I inspected this well location as an evaluation of current conditions and follow-up to previous Compliance Notice issued for missing well identification and release of oil at the battery. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery located on a larger, farm property. The battery was comprised of one 100-barrel, steel production tank, one 50-barrel, steel brine tank, and horizontal mechanical separator. There was a large area of oil staining at the base of the production tank, indicating a prior releases of oil to the ground surface that remained unremediated. There were no apparent active leaks, and the valve-connections at the wellhead were closed. The various elements of the production systems were in disrepair, with inactive chart recorder, and broken doors on the separator unit. Access to the well and battery were afforded by a shared-use, gravel-paved access road that was in good condition. There was no ID signage.

1234691965 2012-07-10 Production Wells Violations Noted On 7/10/2012, I conducted a production well inspection status check. My inspection found the well was a plunger lift unit (rabbit) that was in good condition. The tank battery contained one 100 barrel oil storage tank, with no well information on the identification sign, one 50 barrel brine storage tank and a separator. I noticed no visible spills or leaks. The lease road was in good condition, inspection not passed.
1285508938 2016-04-28 Production Wells Violations Noted I conducted this inspection to follow---105 requiring Big Sky Energy Incorporated to suspend all oil and gas operations in the State of Ohio. My inspection found the wellhead equipped with a 1.5 plunger lift system. The tubing ball valve and flow line valve at the wellhead are open and the domestic tap is open. The electronic controls at the wellhead are set to cycle in six (6) hours. Oil has leaked from the hammer union connection below the lubricator and below the tubing ball valve. No oil is on the ground but these leaks indicate a possible safety or environmental hazard if the connection(s) fail. The well produces to a horizontal separator and one of the doors is lying on the ground in front of the unit. The flow line valve and sales line valve are open at the separator. The 7 day production chart at the separator was changed on 4-21-2016 and is recording production at this time. The tank battery is diked and contains one (1) 100 barrel tank and one (1) 50 barrel brine tank. There is no ID posted at the tank battery or wellhead as required. An inspection of the tank battery revealed the presence of an oil spill that the operator was attempting to remediate. A sump has been dug in the center of the tank battery in an attempt to flush a prior spill into the hole where it could be collected. Stone and soil around the tanks are visibly impacted and the sides of the sump show affected material to a depth of 4-5. Big Sky Energy will be given one (1) week from the issuance date of this notice to perform the following remedial activities: 1) the tanks will need to be steam cleaned and pulled from the battery, 2) impacted stone and soil will be excavated down to clean soil or clay, 3) the contaminated soil will be disposed of at an approved landfill, and, 4) clean soil and stone will be replaced prior to rebuilding the t
1393509014 2014-02-26 Production Wells Violations Noted I was on location today to check compliance of Chiefs Order # 2014-43. My inspection found the well to be equipped with a plunger lift unit. The demco valve and the valve leading to the flow line on the wellhead are open. There is a domestic line producing off of the 4.5 casing. The tank battery consists of a 100 BBL production tank and a 50 BBL brine tank that is surrounded by a proper dike. The heat treated separator has the proper spacing. The flow line valve leading into the separator is currently open. The sales line valve leading out of the separator is currently open. The production chart in the separator shows the well has been produced since the last chart change on 2-19-2014. The lease road is in stable condition. The well is in production which is in non-compliance of Chiefs Order 2014-43. The well needs shut-in immediately.
