0337867321 2015-02-25 UrbanProduction Wells No Violations I was on location for a follow-up inspection in reference to compliance notice #1059347449. During my inspection, I found a trailer positioned near the rental house containing impacted material. The trailer was lined with plastic. A snow, covered path had been plowed leading to the tank battery and the remaining impacted material. The 100 barrel production tank was still full of oil and water. The hatch on the 50 barrel brine tank was frozen shut. The well was shut-in with the flow line ball valve in the closed position. All of the small, plastic containers except one had been shut-in. According to Kurt Kollar, Ohio EPA, Mr. Svetlak has until 2/28/2015 to remove the rest of the impacted material. A follow-up inspection will be conducted by 3/2/2015.
0343324011 2015-03-02 UrbanProduction Wells No Violations I was on location for a follow-up inspection in reference to compliance notice no. 1059347449. During my inspection, I found Mr. Svetlak on location removing the remaining impacted material. Mr. Svetlak was using a bobcat to load the material into a lined trailer. Mr. Svetlak stated the material was going to the Carbon Limestone Landfill in Mahoning County. Mr. Svetlak estimated three (3) trailer loads of material were left on site. I reminded Mr. Svetlak to file annual production records for the well. A follow-up inspection will be conducted by 3/16/2015.
0475847793 2014-12-02 Production Wells No Violations I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found no additional removal of the impacted soil and weeds. The production tank and brine tank were completely full of water. The two containers with impacted soil and weeds were still on location. I called and left a message with Mr. Svetlak to return my call for an update on the oil spill clean-up operations. Follow-up inspections will continue.
0514185335 2017-06-08 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for inspection report 1496943118. At the time of this inspection, Circle M the cleanup contractor for the site was on site with a vac truck to start the process of fluid removal from the impacted well site. Removal of fluid was ongoing throughout the day. I spoke with Mr. Svetlak regarding the continuation of the cleanup efforts until all free fluid was removed from the site. He indicated that the service rig would be on site by either June 9th or June 12th to begin the repair process on the well tubing.

Mr. Svetlak stated clean-up of the site will continue and that the gas leak should be repaired by Thursday, 10/30/2014. Also, he state a sample of the impacted soil was taken to Cardinal Environmental Lab in Youngstown, Ohio to test for Benzene. Once testing is complete, Mr. Svetlak plans to dispose of the impacted soil and weeds at the Carbon Limestone Landfill in Lowellville, Ohio. Mr. Svetlak asked for addition time to cleaning up the location. An extension for the site clean-up was given to 11/14/2014. Follow-up inspection will continue.
0744649042 2017-06-16 Production Wells No Violations I was on location to assess compliance notice #1494865874 and compliance notice #792042042. I found the two (2) storage tanks removed from the site. The separator has been moved into position to be removed. Jon Davis of Circle M is on location grading the well site. All of the oil has been cleaned up. The oil impacted soil and debris has been placed in a pile and will be removed from the location once the disposal containers arrive.
0792042042 2017-06-06 UrbanProduction Wells Violations Noted At the time of this inspection, the well was observed to be equipped with a plunger lift assembly and the had an active gas and oil leak from the tubing and 8 5/8 inch casing. The company was working on the 8 5/8 inch casing to repair the leak when the 1.5 inch tubing split open and started leaking. On site was well owner, Steven Svetlak had applied oil absorbant booms and rags around the well and adjacent area where surface water had been impacted. Mr. Svetlak had plumbed the well directly to the 100 BBL production tank within the tank battery. The tank battery location had a 100 BBL production tank, a 50 BBL steel brine tank and a mechanical separator. The separator was outside of the diked battery. Oil was observed within the dike and outside of the diked tank battery approximately covering a thousand square feet. The 100 BBL production tank had approximately 80 BBLs of fluid in it, so Mr. Svetlak had another 150 BBL tank brought to the site to remove the fluid from the 100 BBL tank. I advised Mr. Svetlak to repair the well and remove all the oil and water from the site along with any impacted vegetation.

0966837214 2017-08-01 Production Wells No Violations I was on location with Trumbull County Inspector Rick Skidmore to follow-up on Compliance Notice #s 1494865874 and #792042042 related to pollution and contamination issues. This inspection found a wellhead equipped with 1.5 tubing and a 4.5 casing head bowl. The valves on the port lines from the bowl are closed and the tubing flowline has been removed. The top of the tubing is equipped with a gate valve and the valve is closed. It was previously discovered that there was a hole in the 8.625 surface casing, which is not currently visible, and was allowing natural gas and oil from the Berea Sandstone to flow outside of the surface casing and contaminate the area around the wellhead. The area around the wellhead has been covered with absorbent pads and the pads are visibly stained. A one (1) inch line has been plumbed from the surface casing port (not visible) to the flow tee, and a line has been run from the flow tee to a 50 barrel steel tank that is spotted west of the wellhead. There is low pressure, natural gas visibly venting from the stack on the 50 barrel tank. Remedial plans are currently being reviewed to repair the leak. The location has been scraped and areas of visible impact still remain along with numerous bent sticks of two (2) inch screw pipe. The contaminated material has been stockpiled northeast of the wellhead and needs to be removed. This violation continues.
1005337726 2014-11-26 UrbanProduction Wells No Violations I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found no change from my inspection on Friday, 11/21/2014. I called and spoke with Steve Svetlak. Mr. Svetlak stated 8 loads of impacted soil and weeds mixed with mulch were removed from the site. I instructed Mr. Svetlak that:

1414069455 2014-10-18 UrbanProduction Wells No Violations I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found Mr. Svetlak and Steve Stewart on location cleaning up the oil spill. Mr. Stewart was working a clearing phragmite weeds in and around the tank battery using a Bobcat Track Hoe. Upon further inspection, I found the 100 barrel production tank and 50 barrel brine tank in good condition. Both tanks had bull plugs on the load lines. There was a dike surrounding the tank battery. I noticed a sump pit had been dug near the well and Mr. Svetlak stated the oil and water would be pumped into the 100 barrel production tank. Also, Mr. Svetlak stated he was working on obtaining sawdust and peat moss to bio remediates the soil. Follow-up inspections will continue.

1494865874 2017-05-15 UrbanProduction Wells Violations Noted I inspected this well location as a follow-up to a previous inspection and associated Chiefs Order #2016-171. At the time of this inspection, I observed the following: a wellhead with plunger-lift production system and associated supply-line appurtenances, and associated storage tank battery. The battery was comprised of a 100-barrel steel, stock tank, and 50-barrel steel brine tank. The areas surrounding the well and battery were comprised of a dense overgrowth of phragmites and other mature, wetlands-type vegetation with large areas of standing water. The existing flow-line valve connections were closed, with the following exception: there was a 1-inch domestic supply connection, with open valve connection displaying 150 psi on an existing gauge. I observed a release of oil surrounding the vicinity of the wellhead, and impacting surrounding surface and floating on the adjacent ponded waters. The oil impacted area was estimated to be 1,000 square feet of surficial oil impact and floating free-phase oil. I walked areas in the apparent down-gradient direction of the wellhead and did not observe visual evidence that oil or oil-impacted surface water had migrated beyond the immediate area of the well and the limits of impact previously described.. I notified ODNR Emergency Response as well as the well owner and advised the well owner regarding the duty to report the release to local state and local authorities. The well owner must begin implementation of remedial measures as soon as possible and complete all remedial and restoration measures by 06/02/17. 1496856509 2017-06-06 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for inspection report 1494865874. At the time of this inspection, the area of impact from released oil surrounding the vicinity of the wellhead and adjacent ponded water within the well location, has not been cleaned up. I contacted the owner Steven Svetlak about cleaning up the area and removing any impacted vegetation and soil for disposal.
