I found Pin Oak Energy has removed the 210 storage tanks and production equipment from the tank battery. I spoke with the landowner and current owner of the well, Gary McIntosh. Mr. McIntosh is satisfied with the removal of the storage tanks and equipment. Mr. McIntosh is good with closing out the complaint.

The two (2) storage tank ladders have been disconnected. Inspections will continual until the storage tanks are completely removed.

1599985902 2016-12-19 Production Wells No Violations I met with Matt Pettigrew on location concerning the well and tank battery. Mr. Pettigrew is the contracted well tender for Pin Oak Energy. I told Mr. Pettigrew:



Mr. Pettigrew stated he would meet with Pin Oak Energy and begin addressing these issues. Pin Oak Energy took ownership of the well on 10/11/2016. Follow-up inspections will continue until all issues are corrected. The well shares the tank battery with permit 2-0406.

1907422622 2017-07-07 Production Wells No Violations I met with landowner, Gary McIntosh, and Jim Pavlic, well tender for Pin Oak Energy on location. I witnessed the isolation of the sales line at the tank battery and shut-off of the circuit break box at the well.

2019154876 2018-04-23 Production Wells No Violations I conducted an inspection on this well site regarding complaint #5336. The well was equipped with a pumpjack and the valves were closed at the time of the inspection. The tank battery location consisted of two 100 BBL tanks and a separator. This well has been transferred as a domestic use only. Separator sales line has been disconnected and the domestic line was plumbed off of the separator to the residence. The three 210 BBL storage tanks at the tank battery, that were required to be removed has been completed. The area approximately 20 feet southwest of the pumping unit, the soil has been removed and replaced with clean soil. I met with landowner Gary Macintosh on site. Mr. Macintosh stated he was satisfied with Pin Oak Energys cleanup and remediation of the well and tank battery locations.
