0177957217 2015-09-22 Urban Drill / Deepen / Reopen No Violations I was on the East Market Street Church# 1 to review compliance with Urban Drilling Requirements. Wildcat Drilling was drilling on fluid at 1069 GL. Mr. Tom Hill noted that the required Danger No Trepassing signage was not in compliance. I contacted Don Nething ( Ohio Valley Energy ) to alert him of the short coming. He said that the signs will be up within hours. A follow up inspection noted that the signage was in place. A pneumatic line that operated the BOP had been briefly remove to recharge Hydril on the mud pump. I requested that the line be immediately reconnected to the BOP and remain operative until the 4.5 production casing is placed in the bore hole.
0255881439 2015-09-14 Urban Drill / Deepen / Reopen No Violations I was on the East Market Street Church # 1 drill site to witness the placement and cementing of the liquid tight cellar . Daley Excavating removed the asphalt, from the parking lot, and dug a 8 X 8X 5 hole, placed 6 mil plastic liner material and centered a 5 X 4 PVC culvert in the excavation. Sidley Ready Mix delivered 4 cubic yards of six (6) sack concrete and placed it in the excavation surrounding the culvert. After curing overnight, 304 gravel mix was placed in the excavation bring up to grade. No violations identified during this inspection.
0265152602 2016-01-26 UrbanProduction Wells No Violations I was on the East Market Street Church # 1 to assess the construction and restoration efforts by the Operator. The well head is completely built and will be operated as a plunger lift well with 1.5 tubing inside 4.5 production casing. The tank battery is complete and consists of a 210 barrel storage tank, a 33 barrel poly brine tank and 2408 mechanical separator. A gas measurement station is also present. The tank battery is being secured with chain link fencing with privacy slats. The restoration is waiting for better weather for the final grade and seeding. The area surrounding the well head will be paved with asphalt. The erosion controls are still in place and effective. No violations identified.

0305422947 2015-09-18 Casing String Witnessed No Violations I was on the East Market Street Church # 1 to witness the placement and cementing of the 11.75 surface casing. Wildcat Drilling drilled a 14.75 bore hole to 230 GL on a fresh water fluid system. As Wildcat Drilling was lowering the 11.75 casing into the bore hole the casing sat down at 120 GL and plugged the casing. Unable to establish circulation the casing was removed from the bore hole. Wildcat Drilling picked up their drill pipe with a 14.75 tricone bit and went into the bore hole to remove the obstruction. Due to the unknown length of time required to condition the borehole I requested notification when they ready to cement the 11.75 surface casing.
0449565585 2016-05-23 Preliminary Restoration Violations Noted I inspected the East Market Street Church # 1 in order to evaluate the status of the preliminary restoration. The well was spudded 9/17/2015 and 1509.072 ( A ) states that the preliminary restoration shall be done within three (3) months of the spud date in an Urban designated setting. My inspection revealed that the seeding of disturbed areas, planting of landscaping stock, repaving the parking lot and replacement of the broken sidewalk had not been completed. I have spoken with Rick Liddle and Don Nething ( both of Ohio Valley Energy Systems ) who said they were have issues receiving timely bids for asphalt and landscaping work. The East Market Street Church # 1 shall have the preliminary restoration completed and approved by DOGRM by 6/24/2016.
0611284930 2015-12-01 UrbanProduction Wells No Violations I inspected the East Market Street Church # 1 on 12/1/2015. During my inspection the tank battery was under construction. The tank battery consisted of a 210 barrel stock tank, 2408 horizontal separator, 25 barrel poly brine tank and a gas metering station. All spacing distances between the various pieces of equipment and the spacing distances to the drilling unit line were in compliance. The fencing requirements were also met. The well is to operated as a plunger lift well. The well head was constructed and awaiting connection to the tank battery. All piping and fittings were visually free from leaks. The 4.5 casing gauge read approximately 650 psi. The tubing could not be read due the gate valve being closed. The annular space between the 8.625 surface casing and 4.5 production casing had a gauge which registered 0 psi. No violations were identified during this inspection.
0760888680 2015-09-18 Casing String Witnessed No Violations Wildcat Drilling drilled a 14.75 bore hole to 230 GL on a fresh water fluid system. Wildcat Drilling ran five (5) joints of 11.75, 42lb/ft, ST&C casing to 199. Petroset Cementing Services (PCS) connected to the casing and established circulation, conditioned the bore hole with 200 lbs. of gel and 50 lbs. of cottonseed hulls. PCS mixed a cement slurry consisting of 270 sacks of Class A cement blended with 3% CaCl. Cement slurry verified by scale at 15.4 ppg. The cement slurry was displaced to 180 GL with 23 barrels of fresh water. I observed approximately 20 barrels of cement slurry returned to surface. Mix water temperature was 76 F and pH was 7. No violations during this inspection were identified.
0830757427 2017-03-08 UrbanProduction Wells No Violations The East Market Street Church # 1 well was previously issued a Notice of Violation ( # 449565585 ) for non compliance of ORC 1509.072 which prescribes the time frame for preliminary restoration approval. Upon re-inspection I found that the well has been brought into compliance. The disturbed area has been reseeded and is growing and stable, the landscaping requirement has been fullfilled and the sidewalk and parking lot has been repved. All requirements, for Preliminary Restoration, has been satisfied.
1033307675 2015-09-27 Casing String Witnessed No Violations I witnessed the cementing of the 4.5 production casing on 9/27/2015. Wildcat Drilling drilled a 7.875 bore hole from 428 to 4307 utilizing a brine based drilling fluid. Os Power Tongs ran 102 joints of 4.5, 10.5, New, ST&C casing, one (1) 10 pup joint with a guide shoe and latch down baffle and 20 4.5 X 7.875 bow spring centralizers. Total tally ran was 4270 set at 4284 KB. Petroset Cementing Services (PCS) placed a plug container on the 4.5 casing with a five wiper rubber plug inside. PCS conditioned the bore hole with 100 lbs. Trisodium Phosphate, 50 lbs. Sodium Acid Pyrophosphate, 100 lbs. Dowflake and 10 barrels Sodium Silicate. After pumping the flush PCS pump a cement slurry consisting of 250 sacks of Thixotropic cement. Displacement was achieved with a wiper plug and 68 barrels of water. The plug was bumped to 1200 psi. and held without any pressure decrease. The float held. No violations identified.
1177395821 2015-09-17 Urban Drill / Deepen / Reopen No Violations I was on the East Market Street Church # 1 to observe the drilling activities being conducted by Ohio Valley Energy. Wildcat Drilling was drilling, on a fresh water fluid system, a 22 diameter bore hole at 63 ( in the Sharon Conglomerate ) for the conductor casing. A walk around inspection revealed no violations.
1442406304 2015-09-11 Urban Drill / Deepen / Reopen No Violations I conducted a site inspection to confirm compliance with Urban Drilling requirements on 9/11/2015. The drillsite is located in the East Market Street Churchs west parking lot. Two deciduous tree were removed to allow for an extension of the drillste and tank battery placement. The required sign is in place at the entrance of the drillsite. Silt fencing was in place on the western and nothern boundries of the location. Silt filter socks was placed additionally on the northern boundry of the drill site. All erosional prevention feature were in compliance with the Urbanized Area Permit Conditions. No violations identified during this inspection.
1443720877 2015-10-12 Urban Drill / Deepen / Reopen No Violations I was on the East Market Street Church # 1 to check the pits, security fencing and rat and mouse holes. The pits has approximately 20 to 24 of freeboard and the liners were intact. Orange safety fencing surrounded the entire location and the pits were isolated with chain link panels. All fencing was effective at the time of my inspection. The rat and mouse hole were not plugged at the time of my inspection. I spoke with Don Nething ( Ohio Valley Energy ) about cementing the rat and mouse holes. He said, they will be filled at the time the pits are solidified. I requested daily monitoring of the location and pits due its urban sitting and potential heavy rainfull over the weekend. Don Nething said, he will keep an eye on it. No violations during this inspection
1471355211 2016-08-16 UrbanProduction Wells No Violations I inspected the East Market Street Church # 1 to assess the compliance with a Compliance Notice issued 5/24/2016 for incomplete restoration. The seeded vegetation was growing, but due dry conditions, at an uneven rate giving a patchy appearance. The silt fencing was ineffective and needs removed. I spoke with Don Nething ( Ohio Valley Energy ) and requested that the silt fencing be removed as soon as possible. Mr. Nething replied that he will remove the silt fencing and bring in soil to fill in along the road surface. Once the moisture content of the soil is high enough Mr. Nething will reseed the area and the time frame is anticipated to be two (2) to three (3) weeks. The gravel border along the parking surface was functioning as a erosional control, it prevented the water from sheeting off the parking lot and causing rutting. The original Compliance Notice remains in effect until preliminary restoration is approved.
1577597769 2015-09-21 Casing String Witnessed No Violations I was location to witness the placement and cementing of the 8.625 intermediate casing. Wildcat Drilling drilled a 11.75 bore hole from 199 to 464 while circulating a brine based drilling fluid. The Berea Sandstone was found from 345 to 368 GL. Wildcat Drilling placed 11 joints of 8.625, 23 lb/ft, ST&C casing and four (4) 8.625 X 11.75 bow spring centralizers. Total tally ran was 428 set at 442 KB. Petroset Cementing Services (PCS) connected to the casing and established circulation. PCS mixed a cement slurry consisting of 200 sacks of Class A blended with 3% CaCl and circulated it back to surface. Displacement was achieved utilizing a top plug and 26.5 barrels of fresh water. I observed approximately 12 barrels of cement slurry back to surface. No violations were identified during this inspection
1754146601 2015-09-09 Urban Drill / Deepen / Reopen No Violations I received notification from Don Nething ( OVE, 330.283.5758 ) that site preparations were to begin on the East Market Street Church # 1 on 9/9/2015. My inspection of the location showed that two trees and chain link fencing were being removed. I spoke with Mr. Nething concerning the pit water and drilling fluid, liquid tight cellar, fencing around the location and pits, drilling the entire well on fluid, silt fencing placement and the required information on the signage. A pre - spud meeting will be schedule prior to the spudding of the well. No violations at this time.
1875674326 2015-09-17 Casing String Witnessed No Violations I was on the East Market Street Church # 1 location to observe the placement and cementing of the 16 conductor casing. Wildcat Drilling drilled a 22 borehole to 93 GL on a fresh water mud system. Two joints of 16, 65 lb/ft, ST&C was ran in the bore hole. One (1) 16 X 22 bow spring centralizer was utilized to center the casing at 41. Petroset Cementing Services ( PCS ) connected to the 16 casing and pumped a cement slurry consisting of 325 sacks of Class A cement blended with 3% CaCl and .5 lb/sack celloflake. The cement slurry was circulated to surface. Mix water temperature was 75 F and pH was 7. Cement slurry returns were observed at surface and numbered approximately 16 barrels. No violations were identified during this inspection.
1946755030 2015-10-30 UrbanProduction Wells No Violations I was on location to check the progress of the above ground well construction. Ohio Valley had surveyors on site to measure the positions of the tank battery equipment to ensure compliance. The well was stimulated on 10/20/2015. The wellhead was constructed to operated as a Plunger Lift well. The 1.5 tubing had 900 psi showing on the gauge and the gauge on the 4.5 production casing had 920 psi . The 8.625 to 4.5 annulus had the required pop-off valve and gauge. The gauge registered 0 psi . I spoke with Don Nething regarding the fencing requirements around the tank battery and well head. The temporary fencing and filter socks are to remain functional until seeded and stable. No violation identified during this inspection.
2144602182 2015-09-16 Urban Drill / Deepen / Reopen No Violations I inspected the activities on the East Market Street Church # 1 on 9/16/2015. The drill site is located in an Urban Drilling designated location. Wildcat Drilling was the drilling contractor. Daley Excavation Inc. was the excavation contractor. At the time of my inspection Wildcat Drilling was fi nishing rigging up over the cellar. Daley started pit construction at 1430 hrs. As the pits were dug the material being removed was sandy fill material. I requested that the sides of the pits be angled back more ( decrease angle of repose ) to achieve stability on the pit walls. The pits were doubled lined with 7 mil liners. No violations identified at this inspection.
