Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1348469928 2018-12-07 UrbanProduction Wells Violations Noted The well was located in a warehousing / truck depot parking lot. Access was good to both the tank battery and wellhead. The wellhead and tank battery equipment was surrounded with a two (2) sided chain link fence and an earthen dike. I. D. signage was posted, current and legible. There was one (1) 210 barrel stock tank, one (1) 50 barrel brine tank and one (1) vertical mechanical separator. The load lines were plugged and secured. I did not identify any leaks with the tank battery equipment. The well was operated as an electric pumping well and was running during this inspection. Again, I did not identify any leaks at the wellhead. I identified several violations of OAC 1501:9-9-05 (A). This section addresses the spacing of the surface equipment. The violations included that the mechanical separator was only 30 from the wellhead, not the minimum 50 as required in OAC 1501:9-9-05 (A) (5). Non-compliant spacing was also identified with the 210 barrel stock tank in that it was located approximately 40 from the wellhead. This is in violation of OAC 1501:9-9-05 (A) (2) in which states that the minimum spacing of the production tank shall be 50 from the wellhead. The Operator / Owner shall move the separator and stock tank to a distance a minimum of 50 from the wellhead as detailed in OAC 1501:9-9-05 (A) by 1/25/2019. Please contact Bob Roush @ 330.690.8837 once the remedial work has been completed for an re-inspection to assess compliance.
