0487182179 2018-09-12 UrbanProduction Wells No Violations I conducted a follow up inspection on the Robert May # 2 to assess the level of compliance with a Notice of Violation ( # 881949394 ) issued 4/5/2017. Access was gained, to the tank battery and wellhead, on a graveled residential driveway. The tank battery was surrounded with an earthen dike and chain link fence. The fence was not locked. The area outside and inside the tank battery was so overgrown with vegetation that it impeded my inspection. There was not any I. D. signage posted at the tank battery or wellhead. I identified , in the tank battery, two (2) in ground concrete brine vaults, one (1) vertical separator and a partial coil of orange plastic piping. I could see that one (1) of the concrete brine vaults was full of fluid. On the outside and south of the tank battery was one (1) 8 X 48 drip vessel attached to the separator and sales. The wellhead was located approximately 75 northwest of the tank battery. The wellhead was surrounded with four (4) panels of chain link fencing and also impacted by the overgrowth of vegetation. The well, previously, was plugged back to the Berea Sandstone from the Clinton sand.. The well construction I identified was 4.5 casing with a two (2) inch tubing head and two (2) inch tubing. A gate valve on the two (2) inch tubing was in the closed position. There was one (1) closed ball valve and one (1) bull plug in the ports of the tubing head. The flow line was disconnected from the wellhead at the choke. The last reported production was reported for the year 1995. The well was not plugged nor was it capable of producing. The Notice of Violation ( # 8811949394 ) issued 4/7/2017 was not satisfied.
0881949394 2017-04-05 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly and the gate valve on the tubing was open, but flow lines and casing valves were closed. The 2 inch flow line plumbed off of the tubing has been disconnected and equipped with a bull plug. The well was fenced and overgrown with trees and brush. The last reported production for this well was in 1995 and from 1996 to 2014, production has been reported as zero. The tank battery consisted of a concrete brine vault and a mechanical separator. There was a drip bottle located outside of the tank battery fence. The tank battery was diked and fenced with an unlocked man door. There was no signage located at the tank battery or well location. The tank battery was very overgrown with trees and brush. The lease access was stable. I tried contacting Seagull Development Corp and the phone number listed in the state database was not valid. The well owner operator shall submit a written plugging schedule to inspector Shannon Mullins at 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. Per OAC 1501:9-11-05(B), and plug the well within 90 days. The well owner/operator will be notified if the plugging is approved or denied.
1326913366 2017-10-02 UrbanProduction Wells No Violations I conducted a follow up inspection to the Notice of Violation ( # 881949394 ) to assess the level of compliance by the Owner / Operator. The lease road was partially graveled to both the tank battery and the wellhead. The tank battery was surrounded with an earthen dike and chain link fence. The man door at the tank battery was not locked. I did not see any I D signage posted. The equipment comprising the tank battery was difficult to evaluate due to the extensive overgrowth of the vegetation. I did identify the following: one (1) vertical separator, two (2) concrete inground brine vaults (full of fluid ), various associated piping and fittings. The was not any indication of leakage from the equipment. A drip tank was located outside of the perimeter of the fencing on the south side of the tank battery. The wellhead was also fenced and overgrown. The wellhead lacked any I D signage. The wellhead was constructed ( surface inspection ) of 4.5 casing with a two (2) inch tubing head packed off on the two (2) inch tubing. A Demco gate valve appeared to be closed as were the side wing valves. The flow line was disconnected and rendering the well incapable of production. A Well Completion Form ( Form 8) indicated that the Clinton sand was deemed dry and the well was plugged back to attempt to produce the Berea Sanstone. The last reported production was for the reporting year 1995. The well was not plugged nor was it capable of producing. The Notice of Violation ( # 881949394 ) isued 4/7/2017 has not been satisfied.
1373396839 2013-07-09 UrbanProduction Wells No Violations I inspected this well to follow up on idle well complaint #4372. Records indicate that the commercial production of this well ceased in 1994 and has not been produced since. Records also indicate that this well was plugged back in 1988 to a depth of 698 in order to produce from the Berea Sandstone. My inspection found the well equipped with 2 tubing and a gate valve. The tubing was set on hangers atop the 8.625 surface casing and was shut-in. The wellhead was fenced and inaccessible due to the dense overgrowth of trees and brush and there was no sign of anything leaking. The tank battery was fenced, diked, and overgrown with trees and brush that restricted access. There were no stock tanks present and only a vertcal separator was apparent inside the fencing. A drip bottle was located just outside the fenced area. The proper ID sign was not found at either the wellhead or the production equipment. I will forward my findings to Summit County Inspector Bob Roush and Supervisor Tom Hill for compliance enforcement.
