0101900055 2007-04-10 Production Wells No Violations I received a natural gas odor complaint from Dominion East Ohio. The complaint originated from a neighboring apartment building. The tubing head ring was found to be leaking. Moore Well Service hammered the ring tighter and the leak stopped. The oil spill remediation at this site has entered the reclamation phase.

0622304693 2003-06-30 Production Wells No Violations CASE 03-020 The Uniontown office received a complaint from Mr. Tom Wilkinson at 2155 Northampton Road, Cuyahoga Falls (330-338-6872) concerning standing water inside of the tank battery for this well. On this date, I met with Mr. Wilkinson and inspected the site. There was no indication of excessive water retention. The tank battery was properly diked and identified. No violations at this time. Mr. Wilkinson was satisfied.
0707411539 2007-03-06 Production Wells Violations Noted I received a call from Jeff Moore with Moore Well Service reporting an oil spill attributed to this well. The 100 bbl stock tank was overfilled. Crude oil filled the containment area, ran out of the containment area via an uncapped six inch drain pipe, across the landowners yard and driveway and into a ravine. Bart Raye with OEPA (330-418-1288) also responded to the spill. An estimed 10 to 15 bbl of oil was released. Moores initial response was to install a siphon dam downstream, start recovering free oil with a vacuum truck and contain the release as close to the source as possible. Impacted soil will be excavated, stockpiled and disposed at Petrocell. The tank battery is too clse to the wellhead and must be dismantled and moved a legal distance.
0724966534 2007-03-14 Production Wells No Violations Moore Well Service is still on site removing impacted soil, removing a dike underdrain and replacing sorbent pads. To date four loads of soil have been disposed at the Petrocell facility in Lodi. It appears that after the stock tank was overfilled, the uncapped 8 diameter, dike underdrain permitted the oil to leave the secondary containment area and impact the surrounding surface area and the creek. The drain will be removed as part of the spill remediation. A replacement underdrain is not permitted.
0737894910 2014-02-03 Administrative Inspection No Violations I received a call on 1/31/2014 from Michelle Damangeo ( 2096 Pinebrook Trails, Cuyahoga Falls, OH ) concerning the Shepherd #2 well. She commented that the Shepherd # 2 was very noisy when running at its cycle at 1:00am and disturbing her sleep. I inspected the well on 2/3/2014 and found that the production cycles were from 9:00 am to 10:00 am and 11:00 pm to 12:00am according to the timer. I also noticed that while locks were in place they were not being used, that the tubing head was leaking, the wellhead was covered with paraffin. I spoke with Mr. Shepherd and made him aware of the problems on 2/3/2014. He said he will have his pumper limit the pumping cycle to daylight hours, grease the pumping unit , fix the leak on the tubing head and perform general maintaince on the well. I spoke with Michelle Demango ( 234.312.5344 ) and informed of the cycle changes and that shouls remedy her problem.

Restoration will include installing the required fencing around the well and the tank battery, topsoil and seeding the affected area and replanting on the containment dike.

1333986639 2012-04-02 UrbanProduction Wells No Violations The Uniontown office received a call from Michelle Demangeont (330-571-2099), at 2096 Pinebrook Trail, reporting a gas odor and discoloration of her water. This address is located east of the Sheperd #2 well which was drilled in 1984 and completed in 1985. The natural gas odor may be connected to two possible sources. First is a small leak that was noted around the wellhead. The odor dissipated quickly and would not likely carry to the Pinebrook address. The second possible source is the tank battery. The pumping unit is operated daily and the natural gas and any produced fluid are dumped into the stock tank with the gas venting to atmosphere from the top of the tank. I advised Dave Shepherd of the problem and suggested that he start with a charcoal filter to control the natural gas odor from the top of the tank. If that is unsuccessful then he needs to install a separator in the production stream to control the gas. The water well provides water to multiple units in the condominium development. As a public water supply, the Ohio EPA would require regular testing of the water and would respond to any irregularities in the analytical results.
1364390351 2013-03-26 Administrative Inspection No Violations The Uniontown office received a complaint from Michele Demangeont of 2096 Pinebrook Trails, Cuyahoga Falls, OH 44223 ( 330.571.2099 ) concerning a gas odor coming from the Shepherd # 2 well behind her residence. She also noted excessive noise coming from the pumping unit. I called Michele Demangeont @ 0355 Hrs. ( 3/25/2013) in order to gather the particulars surrounding her complaint. I inspected the Shepherd # 2 well on 3/26/2013 and found the following: 140 Bbl. Stock tank, drip tank for domestic house hook -ups and a Pumping unit over the well. Absent were a Separator and a gas metering station. I could not detect any natural gas odor nor ( since the unit was not running ) detect any noise. During my inspection I observed that the stock tank was almost full. It appears that since the tank did not have volume enough to allow normal dissipation of the gas stream, when operating the gas by-passed the charcoal scrubber. I called Dave Shepherd ( 330.929.0098 ) and informed him of the situation and complaint. I asked Mr. Shepherd to have the oil hauled in order to create additional retention time for the gas to dissipate. Mr. Shepherd agreed to comply and have the tank emptied.

1912033548 2016-04-04 UrbanProduction Wells No Violations I was on site on 4/42016 in regards to complaint # 5317 about a noisy pump jack behind her residence. The well is the Shepherd # 2 operated as a domestic well by Mr. Dave Shepherd. The well was equipped with an electric pump jack. The complaint centered about the hours of operation and noise levels. This complaint had been voiced before in 2014. I inspected the well on 4/4/2016. The well was not operating at the time of my inspection. I spoke with Mr. Shepherd ( 239.823.8820 ) and requested that he have the well tender lubricate the pump jack and reset the timer to allow the well to run during daylight hours. It appears that eclectrical outages had changed the time on the clock of the timer. I noticed that peat moss had been spread over an area surrounding the domestic meter and drip. The area encompassed an area approximately 3 X 3. I also requested that Mr. Shepherd remove and replace the contaminated soil immediately. I do a follow up inspection to insure compliance and if corrected no further enforcement action would be taken.
2030926415 2005-10-13 Production Wells No Violations Received a call from Mr. Tomlinson at 2155 Northampton Road about a fence that was down around the well on his property. It appears that the operator put a pump jack on this well and the original rabbit well fence is too small. I will contact the operator to discuss extending the cedar fence around the wellhead and the unit.
