0311629838 2018-03-09 Plug / Plug Back No Violations I check the cement level in the 8.625 surface casing and found it at 18 below the top of the casing head. The cement plug was hardened and free from any gas migration indications. I gave approval to affix the identifying plate once the casing was cut a minimum of 30 below grade.
0998923542 2018-03-07 Plug / Plug Back No Violations Chipco rig personnel had previously lowered the 1.5 work string to 3762 and today they opened the 4.5 casing to relieve the casing pressure. J Hawk Water Service had two (2) water trucks on location, one (1) with brine water and the other with fresh water. Chipco pumped approximately five barrels down the 1.5 tubing and then allowed the well to absorb, on vacuum, the brine until the well was balanced. The capacity of the 4.5 casing was 60 barrels and a total of 75 barrels was used to load the hole until it quit taking the fluid on vacuum. Mixed water parameters were temperature was 35 F, pH was 8 and Chlorides were 414 ppm. Chipco mixed a cement slurry consisting of 35 sacks of Class A cement and displaced it to 3300. Prior to achieving full displacement ( 8.3 bbl.. ) the displacement pump was disengaged ( @ 6.5 bbl.. ) to check if the well was on vacuum. The water level in the tub remained static and the displacement was completed to the calculated volume. Chipco removed the 1.5 work string from the well and shut in the well. No violations identified during this inspection.
1159921965 2018-03-06 Plug / Plug Back No Violations Chipco personnel were on location tallying the 1.5 work string prior to lowering it into the 4.5 production casing. I informed that the setting depth of the work string should be approximately 3764 to be compliant with OAC 1501:9-11-08 A (1). I spoke with Chip Smith ( Chipco ) to review and approve of plugging intervals and procedures. I requested to have sufficient saltwater on location to load up the well prior to displacing the Clinton sand plug. No violations identified during this inspection.
1200686433 2018-03-08 Plug / Plug Back No Violations Chipco Well Services opened the 4.5 casing and after a slight puff of gas, the casing was static. Chipco then lowered their ripping tool into the 4.5 casing and tagged the top of the Clinton plug at 3350, approximately 48 lower than the displacement depth. The Clinton sand plug was 336 over the top of the Clinton sand. Chipco tried to initiate a telegraph at 2510 and could not find free pipe until 2224. The 4.5 casing was parted at 224 and one (1) joint of 4.5 casing was removed to place the bottom of the casing at 2182. Chipco mixed a cement slurry consisting of 86 sacks of Class A cement and displaced the slurry through the interval from 2182 to 1897. Slurry density was 15.1 ppg. Mix water parameters were temperature was 34 F, 414 ppm Chlorides and pH was 8. Chipco removed 38 joints of 4.5 casing to 601 and circulated 35 barrels of fresh water to clear the 8.625 surface casing. A cement slurry was mixed and displaced through the interval 601 to 401 using 6.5 barrels of fresh water. The remaining 14 joints of 4.5 casing was removed from the 8.625 surface casing. Appalachian Well Surveys set a cast iron bridge plug at 250. Chipco lowered seven (7) joints of 1.5 tubing to 234 in order circulate the surface plug back to surface. The cement slurry weight was verified, by scale, at 14.7 ppg. Chipco removed the seven (7) joints of 1.5 tubing and then topped off the 8.625 surface casing. The casing was static without any indication of gas migration.
1412855546 2019-01-31 Final Restoration No Violations My inspection found the former well and tank battery sites are being developed for a residential allotment. A landowner waiver was received on 01-14-2019 at the Uniontown office of the ODNR Division of Oil & Gas. The Landowner waiver waives the removal of any pipeline. The final restoration is passed.
1520438620 2018-03-05 Plug / Plug Back No Violations Chipco Well Service was the plugging and cementing contractor for the well. The well was completed as a pumping well. Chipcos service rig was removing the rods and 2.375 tubing from the 4.5 production casing. The casing was reported to have approximately 250 psi. I requested that a apron be placed around the wellhead to catch the paraffin coming off the rods. The rig operator indicated that the fluid level was down at ~ 2500 per the fluid level on the tubing. The operator indicated that a 1.5 tubing work string would be employed to set the cement plugs and a wireline bridge plug would also be set. No violations found during this inspection.
1993065629 2018-03-07 Plug / Plug Back No Violations With Bob Roush (MRI-3), I am on location to witness the plugging of this well. CHIPCO is the plugging and cement contractor. The hole was loaded with brine and went on vacuum. A total of 65 barrels of brine were loaded in the hole. The bottom plug was placed.

Mixing water: pH 8.0, Chlorides: 414 ppm; Cement weight 15.0
