1058824752 2017-11-01 Production Wells Violations Noted The well was accessed on a paved driveway which also served the residence. The tank battery was comprised of one (1) 100 barrel stock tank and one (1) 2410 horizontal separator. The spacing between equipment was compliant. The tank battery was surrounded by an earthen dike and chain link fence. The Identification signage did not reflect the current Owner. The load lines were plugged. I did not identify any leaks or contamination in the tank battery. The wellhead was surrounded by a chain link fence. The well was configured to operate as a plunger lift well. Well construction ( as seen at surface ) was 8.625 surface casing, 4.5 production casing and 1.5 tubing. All the valves, controlling the flow towards the flowline, were closed. A domestic gas manifold was connected to the 4.5 casing with the two (2) inch valve in the open position. The domestic gas manifold consisted of one (1) Big Joe regulator, one (1) 6 X 48 vertical drip tank, one (1) Little Joe regulator and one gas meter. Gauges on the casing and house line read 110 psi and 11 psi respectively. The well was located approximately 530 to the northeast of the residence. Two (2) issues were identified at the wellhead. First, the 8.625 X 4.5 annular outlet ( casing head ) did not have a valve or other control measure. Secondly, there was a hammer union, on the flow line side of the wellhead, that was leaking and causing oil staining on the ground. Both issues are in violation of 1501:9-9-05 (A) (8), which states All surface equipment must be pressure rated to withstand operating pressures to which it is subjected. Once the transfer is finalized the signage requirement shall be compliant with the Urbanized Areas criteria ( 1501:9-9-05 ( E ). In order to comply with OAC 1501:9-9-05 the Owner shall 1.) place valve or other control device on the 8.625 X 4.5 annular outlet on the casing head, 2.) stop the oil leak on the hammer union and place Urbanized signage reflecting the current Owner on the tank battery and
1515173619 2018-01-05 Exempt Well Pre Transfer No Violations I conducted a Compliance notice follow up inspection to assess the if the deficiencies detailed in Compliance Notice # 1058824752 dated 11/1/2017 were corrected. The open two (2) inch port in the 8.^25 X 4.5 wellhead had been closed with a bull plug. The leaking hammer union on the wellhead had been fixed and did not exhibit any evidence of leakage. The well was brought into compliance and the Compliance Notice ( # 1058824752 ) was satisfied. Once the ownership of the Ciesick Unit # 1 well is transferred the new Owner shall post Urbanized Area compliant signage within 30 days of said transfer. The I.D. signage shall meet with the requirements of OAC 1501:9-905 ( E ). Please call Bob Roush ( 330.690.8837 ) for re-inspection once the new signage is posted.
