1013474420 2016-05-11 Orphan No Violations Met with landowner, Slobodan Pavlovic, discussed orphan well process and viewed well site, gathering local information and took video of well.
2111200127 2016-03-17 Orphan No Violations A complaint ( # 5279 ) was received by the Uniontown office regarding dead vegetation in the area of an old plugged well. The landowner ( Slobodan Pavlovic 4562 East Boston Road, Richfield, OH, 44286 ) had noticed the dead vegetation over an extended period of time and occasionally smelled what he described as gas. Mr. Pavlovic excavated the area and found casing with the fluid bubbling inside. He centered a section of 15 black plastic culvert over the casing creating a small cellar, then backfilled the excavation around the outside and placed a vented lid on top of the culvert. I inspected the culvert pipe, found it full of water and actively bubbling. I told Mr. Pavlovic that the water needed to be pumped out in order to do a complete inspection. A small electric submersible pump was lowered onto the culvert which removed the water and revealed a 8.625 casing 40 below grade. The size of this casing was determined by a piece of the casing that the Landowner found during excavation. The inside of the 8.625 casing was actively bubbling with gas without any indication of gas on the outside of the casing. My gas detector indicated the presence of methane. I tested the fluid in the casing using a low range Quantab and found the Chlorides level to be 46 ppm. A probe was inserted in the casing and an obstruction in the casing ( cement top ? ) was encountered at 46 below grade. The well is not accurately located on the Division Well Locator Site. It is approximately 86 NW of the well spot location on the Division map. The casing is located in a grass lawn with a border of pines 20 to the East. Mr. Pavlovics water well is 220 West of the casing and has not exhibited any changes in quality or quantity, per the landowner. A review of ground water data in the area indicates that water is found in varying depths from 46 to 140. The predominate aquifer is the Sharon Conglomerate member of the Pottsville Formation. The landowners and a neighbors residence are approximately 20
