1081340000 2004-04-06 Solution Mining Project No Violations Completed the Mechanical Integrity Test on the 5-1/2 casing in this solution mining well. Ran a Halliburton RTTS retrievable packer on 2-3/8 tubing. Set packer at 2663. Pressure up to 150# and found a leak at the tubing head. Tighten and retest to 500#. Had a small leak in the packing of one the old valves. The test had to be done through the old valves because of the connection sizes. This leak probably accounted for the 3% pressure loss (500 psi to 485 psi) after the 15 minute test. All of the valves will be replaced before the system is placed back into operation.

Initial - 500 psi 5 Minute - 500 psi 10 Minute - 495 psi 15 Minute - 485 psi TEST PASSED.

1134131424 2010-01-12 Solution Mining Project No Violations The required 5 year MIT is still on hold at the request of the City. They are waiting for the Moore Well service rig to pull the tubing and set a packer to conduct the test. On this date they were producing some brine which was being discharged into their containment pit.

1435441880 2015-04-23 Solution Mining Project No Violations I was on the City of Cuyahoga Falls # 1 for the five ( 5) year Mechanical Integrity Test. Moore Well Services ( MWS ) set a Baker Model 81 packer at 2701 ( mid element ) on 2.375 tubing and began the pressure test using a triplex pump. Allowable injection pressure for the well is 400 psi. A test manifold isolated the well from any possible bled off back through the pump and plumbing. A Barton chart recorder and a digital gauge monitored and recorded pressures at timed intervals. The annular space between the 5.5 casing and 2.375 tubing was monitored for any pressure leakage. The initial pressure was 527 psi ( @ 0838 hrs. ) followed by 525 psi ( @ 0843 hrs. ), 520 psi ( @ 0848 hrs. ), 518 psi ( @ 0853 hrs. ) and 513 psi ( @ 0858 hrs. ). Test duration was fifteen minutes, during which a 9 psi decline ( 1.7 % ) occurred. The well passed the five (5) year Mechanical Integrity Test.

During this inspection, brine was being pumped into the holding pond and City trucks were loading for road application.

1817416591 2016-08-11 Solution Mining Project No Violations Moore Well Services (MWS) was on location to change out the 2.375 injection string after the salt density had dropped. MWS removed the tubing badly corroded which was being crushed where the power tongs bite into the tubing. The bottom two (2) joints of tubing had been sheared off, possibly due to cavern collapse. Moore Well Services ran a used string of 2.375 tubing consisting of 89 joints and one (1 ) mud anchor. No violations observed during this inspection.
