0256771592 2017-05-31 Plug / Plug Back No Violations I was on location to check the integrity and height of the top plug in this recently plugged and abandoned well. I rigged up a weight to my tape measure spool and verified the cement top 98 below ground level and the top of the 10.25 drive pipe. The cement was solid and I observed no leakage from the plug. The developer of the property will be lowering grade by five (5) feet at this location and the casing will be cut off 36 below that grade. Topping off the casing will not be necessary and the casing may be cut-off. No violations.
1053506488 2006-04-28 Production Wells No Violations Status check inspection. The well, identified as the Gilbert #2, is owned and operated by Harry Gilbert. His address is 3154 Graybill Road, PO.Box 415, Uniontown, Ohio, 44685. His phone number is 330-699-3325. I spoke with Mr. Gilbert and asked him to replace the faded well ID signs and set a secondary containment dike around the tank battery.
1495647501 2017-05-24 Plug / Plug Back No Violations I was on location at 9:49am to witness the plugging operations commence on this Clinton Sandstone well. Nicks Well Plugging (NWP) was on location to conduct the plugging operations. They spotted the service rig and pipe trailer northwest and southeast or the wellhead respectively. An earthen pit was constructed on the north side of the wellhead and I witnessed the lining of the pit. The valve on the 1.5 tubing was opened and blown down in an effort to recover the plunger (if present) and it was not encountered. The 4.5 production casing valve was opened and blown down prior to conducting any more work. I asked NWP to run a sinker bar down the 1.5 tubing to verify the tubing depth. The depth was determined to be 4270 just above the Red Clinton Sand. NWP ran two (2) 32 joints of tubing into the hole and fill-up was encountered 20 into the second joint and that joint was laid down. NWP packed in the tubing and rigged up. They began pumping down the tubing and verified that the tubing was clear. They pumped 30 barrels of water and then mixed and pumped 8.4 barrels of cement slurry. The slurry was displaced to 3775 with 9 barrels of water and they broke off and began pulling the tubing from the hole. NWP fitted the 4.5 casing with a 4 x 2 reducer and 2 ball valve, and NWP shut the well in for the evening. Operations will continue on 5/25/17. No violations.
1495724097 2017-05-25 Plug / Plug Back No Violations I was on location this morning with Nicks Well Plugging (NWP) to continue with the plugging operations of this Clinton Sandstone well. The valve on the 4.5 production casing was opened and there was no pressure or gas flow from the casing. NWP rigged up, loaded the hole with 49 barrels of freshwater and pressure tested the casing at 250psi for five (5) minutes to verify casing integrity. They ran in the hole with the knife tool and jars and tagged the bottom plug at a depth of 3830. They came up the hole and ripped the 4.5 casing at a depth of 2960. They pulled the landing joint and three (3) joints of Range-2 casing to a depth of 2860 and rigged up to the casing. They broke circulation and cleaned up the 6.25 hole and then mixed and pumped 11.5 barrels of cement slurry. The slurry was displaced to 2560 with 41 barrels of water. Due to scale and build-up on the casing, the casing tongs would not bite on the pipe and NWP had to chop-saw the joints as they pulled out of the cement. The teeth on the power tongs were replaced and casing recovery was expedited. NWP recovered 91 joints of 9.5lb/ft., Range-2 casing and the stubbed landing joint. NWP cut off the top of the 10 drive pipe to have access to the 6.625 casing and no 8.25 surface casing was found between the two (2) strings. A spud bar (7 in length) was run into the annulus between the strings and no casing was encountered. Plugging operations will proceed under the assumption that the 8.25 surface casing was removed from the well after drilling completion in 1964. NWP cut holes in the 6.625 casing, inserted a steel bar, and attempted to pull the 835 casing string. The rig pulled 65,000lbs on the string several times and could only stretch the casing 10-12. The casing would not move and a decision was made to set a 200 plug (100 in & 100 out) at the bottom of the casing string on 5/26/17 and shoot the 6.625 casing off 50 below the 10 drive pipe at 250 and attempt to recover the pipe and plug the h
1496155005 2017-05-30 Plug / Plug Back No Violations I was on location with Nicks Well Plugging (NWP) to finish plugging this Clinton Sandstone well. The hole was loaded with freshwater and the well continued to take fluid. NWP ran in with 8 joints of tubing to the top of the 6.625 casing stub at 250. They broke circulation with 20 barrels of water mixed with 400lbs of bentonite gel and 120lbs of cottonseed hulls. They then mixed and pumped 31.5 barrels (150 sacks) of cement slurry and cement circulated to surface. The tubing was pulled and the cement had dropped out of visual site. The cement was given a chance to thicken and they ran in with a weight on measuring tape and tagged the top of the cement at 72 below ground level. After an hour, the cement was confirmed at 72 and was not dropping. NWP gravity fed 7.6 barrels of cement slurry into the 10.25 drive pipe and the cement circulated to surface and held. I will check the plug later this afternoon. NWP rigged down, emptied and closed the circulating pit, and moved to the Gilbert #6 (20427) 0.25 miles to the southeast to begin plugging operations. I checked the top plug later in the afternoon and the cement had fallen approximately 10 and had some water on top of it. I will verify with measuring tape and a weight tomorrow (5/31/17). No violations.
1496856215 2017-05-26 Plug / Plug Back No Violations Nicks Well Plugging was on location to plug the above well. (Plug #3 935 to 735). They broke circulation with 10 barrels of water follow by 40 sacks of class A cement. They displaced it with 40.7 barrels water. They pulled the 2.375 tubing. Appalachian Well Service shot the 6.250 casing at 253. ( Plug # 4 259 to surface NWP ran 259 of 2.375 tubing back in the well to set the next plug, they attempted to break circulation no fluid to surface. We spotted a 30 sack plug with 1 bag of Multiseal LCM and pulled the tubing out of the well and shut down until 5/30/2017.
