1. I checked the 2 diameter vent to the west of tree with my gas meter. The natural gas readings are 4% LEL and there were no CH4 IR readings 2. I also checked the 8 diameter vent to the east of tree with my gas meter. The natural gas readings exceeded the meters LEL limits and the CH4 IR is 86 - 90 PPM. I checked with the property owner and contractor (Moore) for the the depth that the vents were set. They stated that both vents were set in a gravel lens that was encountered at a depth of 8 to 9 and appeared to be the conduit for the well gas. The bottom 4 of both vents were slotted and set in this gravel bed. I also asked the contractor for a sketch showing the grid pattern/size and make/model of gas meter that will be used to try to locate the leaking well. I suggested that the property owner contract a Surveyor to plot and physically stake these well locations (153-6-0284 and 153-6-290) using the GPS from the ODNR data base. |

- none detected. The rig crew was getting area around the well casing cleaned up and the venting equipment (drip bottle and drip drum) removed. The crew placed flow lines from the mud pump and half tank to well (using 3 diameter high pressure steel lines. These lines were tethered at each hammer union. The Rig Supervisor brought additional Orange fencing to be placed in the road area near the pond. The crew doing a maintenance check on the power tongs and mud pump. The water truck arrived to transfer fresh water from middle pond to frac and half tanks (total of 7 loads approximately 700 bbls). The tri- They used 4.5 diameter casing as flow line from well to half tank and installed timbers to support the elevated portion of the flow line. The welder fabricated a baffle in the end of the 4.5 casing going into the half tank so it would blow down into the tank. Crew left site 04:00pm. Office time: 06:00 AM to 09:00AM - 3 hrs. On site time: 09:30 AM to 04:00 PM - 6.5 hrs. |

1400503382 | 2013-08-09 | Orphan | No Violations | On 8/9/13, I responded to a complaint of natural gas odor along Kinross Lake Parkway in front of the National Interstate Insurance Company (NIIC) building at address 3250 Interstate Drive. I met with Bill Snyder (NIIC Operation Manager), a field inspector from Dominion East Ohio and the Assistant Fire Chief of the City of Richfield. I received a phone call from Arthur Hammond of Dominion East Ohio. He stated that gas samples were collected on 08/06/2013, from the Service Meter and several points along the Dominion sales line and submitted for analysis. This sales line runs south to the their main line, which runs east/west along the north side of Kinross Lake Parkway.
I contacted the DOGRM County Inspector Bob Roush for assistance. I then plotted the GPS coordinates of the wells offset of the affected area (P# 2-0284, 2-0451 & 20290), obtained from the DOGRM data base, to create location map (see orphan folder). I then used my GPS unit to locate these wells on the property and I marked these with orange flags. None of these wells plotted within the affected area!. I then used my magnetic locator and walked the affected area and found one high magnetic reading approximately 6 to the east of the 6 tree. I advised Bill Snyder to make arrangements to excavate this area and to call me with the scheduled date and time. |

Today they plan to start east of the tree and work their way to the east to the landscaped island. During excavating I used our magnetic locator to check for metal readings and I obtained a response at the northwest corner of the island. The contractor excavated at the metal reading to a depth of approximately 3 where a large rock was encountered. The contractor removed the rock and found open 8.5 diameter surface casing. The collar was badly corroded and had to be cut off and replaced with an 8.625 collar. A vent will be attached to the new collar and the well will be monitored until arrangement can be made to plug this well. The open well bore was measured to a depth of approximately 120. I checked the 8 diameter vent with my meter and found:
- LEL maxed out - 87-90% CH4 IR. I also checked the existing 2 diameter vent with my meter and found: - 3% LEL - 0 % Ch4 IR |

Weather: Overcast with intermittent rain temps in 70s, winds southwest to northeast. Upon arrival I noted a slight gas odor in the parking lot area. I used my Sperian gas meter to check the parking area and a 40 radius around the well for natural gas with no gas readings detected. The three rig hands arrived at 07:30 AM, followed by the rig operator at 07:45 AM and the site supervisor shortly after. They finished leveling the work area pad and then staged the mud boats in the access route, which will be in the lawn just east of the well, until needed. They continued to off load additional equipment, erosion control and spill containment. The crew proceeded to place silt bags over the grates of the storm drains and placed a sorbent boom around the top of each grate. The contractor proceeded excavate the area to the east of the well for the 20x60 mud boat that the rig will sit on. The rig arrived and was staged on the access road until site preparations are completed. Richfield Fire Chief, Richfield Service Department and Police Officer Chris Fyffe stopped and were updated on operation status. Excavation was completed around the well casing to set the tin horn (cellar) and several of the shrubs near the well were removed and placed to the east of the landscaped island. |

Weather: Clear Skies Temp 80s, wind direction from southwest to the northeast. Upon arrival I proceeded to check the well area with my Sperian gas meter as there was a gas odor present around the well casing. I also checked the parking lot area with my meter - no readings in either area. The contractor advised me that since they hit a pocket of natural gas he wants to reconfigure the well control head to accommodate a different air head rubber, so that a bigger (6.25) bit can be used for the clean out procedure. They also changed out several of the high pressure lines from control head to the mud pump and added a rubber suction line from the bottom of the 400 barrel tank to make quicker access to add fluid to the half tank. While waiting for fittings and high pressure line to arrive the crew proceeded to build a platform using planking for the tong operator to work off of. Upon arrival of supplies the crew changed out the high pressure line, reconfigured the suction line on frack tank and changed out the air head body on the well head control.

Crew is on standby and waiting for additional equipment (scaffolding) to arrive. The tool pusher informed me that Bill Snyder advised him that they have been having troubles with their fiber optic line and that Windstream is on their way to diagnose the problem. Rig supervisor arrived with the scaffolding, off loaded it and the crew proceeded to install it around the well casing head for a safe elevated work area.

Weather: Clear Skies, Temp 80s, wind direction from southwest to the northeast. Upon my arrival I noticed a slight gas odor around the well casing, I used my Sperian gas meter to check a 40 radius around the well and the parking lot for gas readings - none detected. Crew set up the power swivel and to be used on the 2.875 diameter tubing with a 7.875 tri-cone bit. They set up a sparking unit at the end of the 4.5 diameter flow line to ignite any natural gas produced by the well. The main control unit was not working and they had to get a new one. The crew had trouble loosing up bolts on well head because were not lube when put away last time. The 7 diameter well head was set on the casing. The crew rigged up 7.875 tri-cone bit on the 2.875 diameter tubing but they were unable to get bit through the well head. The control head was marked 8 but no one checked if this was ID or OD. It was determined that it was 7 ID (inside diameter). They also had difficulty getting the tubing collars to go through the airhead rubber. They had to figure out a way to trim the airhead rubber so tubing would go through. Note that the air head rubber is made of very hard rubber and it takes a lot of weight is needed to get the collars to go through. The crew changed over to a 6.5 tri--cone bit. **I had to leave site and Inspector Bob Roush was on site to document operations while I was gone. |
