0059095868 2014-03-21 Preliminary Restoration No Violations During my inspection I found a pump jack well with all proper gauges and bull plugs. The lease road is stable and there is I.D. signage on the fencing surrounding the wellhead. The tank battery is fenced and is shared with #25718 and #25788. The tank battery contains five 100 bbl production tanks, three 50 bbl poly brine tanks, mechanical separator and has been surrounded by a new SPCC dike. There are no violations at this time and preliminary restoration is passed.
0099728215 2017-06-15 UrbanProduction Wells No Violations I conducted a status check inspection on this well. The well is equipped with pumpjack with an electric motor. The well is fenced in with a lock gate. The diked tank battery consist of four 210 barrel tanks, two plastic brine tanks, two horizontal separators and 2 meters. The tank battery is fenced with two gate entries which are locked. Charcoal filters and spark arrestors are installed. There are two pick-up lines which have plugs in them. The oil pick up line is locked and the brine pick up line is not. I contacted pumper Tony Carlisle he advised me he would get a lock on it. The tank battery is shared with wells 151-25797 & 25718. The tank battery and well are legibly identified.

0953241188 2015-11-17 Work Over Reconditioning No Violations I was on location to check the status of the workover of this well. The Jwell service rig is on location pulling tubing and rods from the well. A snowflake has become disconnected and lodged inside the collar of a joint of tubing. There is also likely a stuck pump on this well. The pump will be pulled from the well and rebuilt before the production equipment is reinstalled and the well put back into production. There are no violations at this time.
1200957047 2013-05-15 Production Wells No Violations I conducted a status check to observe the clean-up of the Roy Klick # 1. Dailey Excavating was loading the contaminated soil into two 25 yard roll-offs supplied by J&J Disposal. Material will be hauled and diposed at a Kimble landfill. New soil and stone was leveled to grade. I observed no violations upon this inspection.
1368188088 2013-05-08 UrbanProduction Wells No Violations I inspected the Roy Klick # 2 upon notification from Dave Matak that the Stuffing box rubbers had failed and an oil release had occurred. I observed that the failure had caused a spray pattern that had contaminated the soil in a Northeast direction for approx. 20 and saturated the immediate area of the wellhead and pumping unit. The area contaminated approximates a 20 x 15 rectangle. I noticed that Peat- sorb was applied to the affected area. I ordered that the fence be wiped down along with the pumping unit, that the affected soil be removed down to the yellow clay and disposed of properly and document disposal to the Inspector. New soil and stone would then brought in and placed back in excavated area to grade. It was later related that the failure was subsequent to the well being treated with a Solvent. The total amount of fluid released appears to be less than 1 barrel.
2126412711 2013-05-09 Production Wells No Violations Upon my inspection of the Roy Klick # 2 location I found Dailey Excavating hand wiping the Pumping unit, wellhead and associated piping and fittings and the north fence panel with a solvent and oil absorbing diapers. Dailey then began excavating the affected area down to the yellow clay. This excavation encompassed an area of 20 x 15 and 6 to 12 deep. It is estimated that approximately 25 tons of material was removed and will be replaced. This contaminated material was placed on a pit liner and encapsulated and a dike was built surrounding it. Operator is waiting analysis to determine final disposal. Such analysis and manifest shall be provided to the Oil and Gas Inspector.
