0918741992 2015-03-10 Production Wells No Violations I was on location to follow up to complaint #4957 regarding natural gas venting from a domestic water well. The lease road is stable and leads up to a plunger lift well with house gas connection. The well is fenced in with I.D. signage. The wellhead is equipped with a 8.625 diameter surface casing valve and the annulus does not show any built up pressure at this time. The tank battery has proper I.D. signage. The tank battery is surrounded by a dike, contains a 100bbl production tank, 50bbl brine tank, horizontal separator and meter run. There are no violations at this time.
1159427059 2018-08-09 Production Wells No Violations I did a status check inspection on this well. The well is equipped with a plunger-lift system. The tank battery consists of a steel production tank, a steel brine tank, a horizontal separator and a meter. The well is legibly identified. No compliance issues are noted at this time.
1205793258 2008-03-18 Production Wells No Violations The Uniontown office received a complaint call on 3-17-2008 from Brad Clendenin (330-484-0388) of 1317 Mattie, Canton, OH, regarding a gas leak located under his driveway. I called Brad Clendenin on 3-17-2008 and left him a message. He returned my call that same afternoon. He said that he and his family had detected an odor several weeks ago and could not determine the source. He had his septic tank pumped out and the odor still persisted. He also stated that on Saturday, 3-15-2008, the odor was still present. Brad and his wife were sick from the fumes and the fire department was called about the odor. The fire department arrived and suggested that Brad and his wife go to the hospital to get checked out. Everflow Eastern was contacted about the leak. George Strawn (Everflow Easterns field superintendent, cell # 330-727-1614) arrived and found that the leak was from Everflows gas sales line. The wells connected to the sales line were shut in and the leak was stopped on the evening of 3-15-2008. I inspected the site on 3-18-2008. There were no violations observed at the wellhead or at the tank battery. I called Kathy Furgiuele (Everflow Eastern 330-533-2692 ext. 29) and discussed the situation. I also called George Strawn and discussed the line leak. George said that the gas sales line will be repaired on Thursday, March 20. I will follow-up with Everflow Eastern to make sure that the gas sales line has been repaired. A copy of this inspection report will be sent to Kathy Furgiuele.
1206137987 2008-03-21 Production Wells No Violations Everflow Eastern has repaired the gas sales line leak by pushing plastic line thru the existing steel line.
1689132799 2014-09-24 Production Wells No Violations I was on location to follow up on complaint #4802 regarding black in the water well and tremors at a home located at 1517 Mattie Street SE, Canton, Ohio. The tank battery contains a 100bbl production tank, 50bbl brine tank, horizontal separator and an ID sign on both the tank battery and wellhead. The tank battery is fenced in. The well is a plunger lift well with 190psi on the tubing. The 8.625 diameter casing is buried and the annulus is not accessible. The well and the tank battery are in good condition and appear to have no mechanical issues.
