0239108864 2015-06-23 Production Wells No Violations I was on location to check the status of cleanup for this well. Sutton Pump has removed all oil and contaminated soil from the tank battery. 15 tons of stone have been installed and a 50bbl tank has been put in the dike and plumbed into the separator. The compliance notice for contamination has been resolved. The compliance notice for production submission and well plugging/ownership issues is still open. PCC is currently gathering paperwork to have the well properly registered.
0713654046 2018-11-23 Production Wells No Violations My inspection found the well has not been plugged as requested by the Compliance Notice issued on 4-28-15 by Fred Romeo. The well has no Annual Production Report Submitted for 2017. The record shows no Plug and Abandon permit being issued at this time. I spoke previously with Patrick Doyle (Range Resources), he advised me Range Resources would be reviewing their ownership of this well.
0965811951 2018-05-21 Production Wells No Violations I did a compliance follow-up inspection on this well. The well is not plugged, legibly identified and no annual production has been reported. The well is equipped with a plunger-lift system. There is a domestic gas connection which provides gas to the residence of Tim & Rhonda Keiser. The tank battery consists of a steel production tank and a disconnected horizontal separator. I spoke with Tim McConaha (Enervest) about metering and reporting the domestic gas production.
1522249944 2018-03-28 Production Wells No Violations I did a compliance follow-up inspection on this well. The well is not plugged, legibly identified and no annual production has been reported. The well is equipped with a plunger-lift system. The tank battery consists of a steel production tank and a disconnected horizontal separator. The well is producing - there is a viable domestic gas connection at the well. I spoke with adjacent landowner Tim Kiser and he advised me he has been using the domestic gas from this well for more than 20 years. I called Tim McConaha (Enervest Superintendent), He advised me the well is owned by PCC and Enervest has been in contact with PCC about transferring the well to PCCs ownership. I spoke with Tim McConaha about legibly identifying well and metering the production for reporting purposes of the well since it is producing domestic gas.
1537365156 2018-09-18 Production Wells No Violations My inspection found the well is not plugged or legibly identified and no annual production reports have been submitted as requested by the compliance notice issued on 04-28-2015. The pollution and contamination cited in the compliance notice was cleaned-up by Sutton Supply - reference Inspection report dated 06-19-2015. The record shows no annual production reports have been submitted since 1999. The well is equipped with a plunger-lift system. The production is shut in. There is a domestic gas connection which is being used by an adjacent landowner for domestic purposes. The tank battery consists of a steel brine tank and a disconnected horizontal meter. Ownership of this well has been changed to Range Resources - Appalachia LLC on 05-21-2018.

1630430463 2015-04-27 Production Wells Violations Noted I was on location to perform an inspection of this production well. The lease road is a shared residential driveway and graveled to the tank battery and wellhead. The tank battery contains a 210bbl production tank, 50bbl brine tank and horizontal separator. There is a 45x15 oil and brine spill in the tank battery which is being held within the dike. The identification on the tank battery is not legible and is listed as Range Resources. The plunger lift well is shut in, and contains a house gas connection. No production has been submitted for this well since 1999 when the well was owned by Range Operating Co. I have contacted Enervest and required that the spill and the leaking equipment be removed from the tank battery and any free product properly cleaned up. This well is incapable of production and shall be plugged per Ohio Revised Code 1509.12(B) within 180 days.
1639774816 2015-06-19 Production Wells No Violations I was on location to witness Sutton Supply finish the cleanup for this well. The dike has been sucked down and surface oil removed. The tanks have been removed from the location and the dike has been scraped clean and the soil disposed of at the Kimble Landfill with manifest created. The dike was steam cleaned and 15 tons of stone have been brought onto location to fill the dike. A new 50bbl tank is on location to be installed in the dike. The ownership of the well is still be determined. Enervest claims that PCC Airfoils of Minerva, OH are the owners of the well. The cleanup is being paid for by PCC Airfoils and the paperwork for the transfer of the well is being completed.
