1136187654 2015-06-16 Production Wells No Violations I was on location to follow up to a spill that occurred on 6/2/2015. The sales line for this well

broke and released approximately 5 gallons of crude oil into a ditch that leads into Nimishillen Creek. At this time the ditch is free of any oil and the sales line has been fixed. There is no free oil in the ditch or Nimishillen Creek. The booms that has been placed in the creek have been removed. There are no violations at this time.

1433258890 2015-06-02 Production Wells No Violations I was contacted by Kyle Fink of Enervest at 10:15am indicating that the sales line for this well has broken and released approximately 5 gallons of crude oil into a ditch that leads into Nimishillen Creek. The Ohio EPA, LEPC and local fire departments were contacted and informed of the spill. A J-Hawk vacuum truck was on location and removed all the free oil from the creek. The sales line was clamped and is no longer leaking. Adsorbent pads were used to remove free oil on the creek surface and covered vegetation was removed for disposal. Oil adsorbent booms were installed in the ditch to catch and control and free oil that may have been missed. The lease road for the well is a shared agricultural drive and is stable. The tank battery is shared with well #23056, is surrounded by a dike, is properly identified and contains two (2) 210bbl production tanks, two (2) vertical mechanical separators, two (2) chart boxes and a concrete brine vault. The gas driven pumpjack is identified and is in good mechanical condition. The booms will be left in the ditch and monitored daily to make sure the oil cleanup is complete.
1434369384 2015-06-04 Production Wells No Violations I was on location to follow up to the sales line leak for this well that has released approximately 5 gallons of crude oil into a ditch that leads into Nimishillen Creek. The sales line was clamped and is no longer leaking. Adsorbent pads were used to remove free oil on the creek surface and covered vegetation was removed for disposal. Oil adsorbent booms were installed in the ditch to catch and control and free oil that may have been missed. There is a small amount of oil still pooling in the ditch. Enervest has been checking and removing the oil twice a day. The booms are still present in the ditch and have prevented any oil from leaving the ditch. No oil is currently leaving the ditch. The booms will be left in the ditch and monitored daily to make sure the oil cleanup is complete.
