0933263604 2015-09-08 Production Wells No Violations I was on location to follow up to complaint #5113. An anonymous phone call was received September 7, 2015 at 4:06 pm to the Uniontown Office. The caller stated near 3750 Farber Road, Magnolia, Ohio 44643 there was a well that was leaking oil from the pumping unit and that the oil was going down a hill. Terry Whitehair (Emergency Operations and Response) and myself responded to the location and identified the leak to be coming from the polishing rod of the pumpjack well. The pumping unit was running but the polishing rod bushings had failed and was spraying oil from the wellhead. The well was shutdown and Enervest had crews respond to the scene immediately. Approximately 15 gallons of crude oil migrated approximately 50 toward the hillside, but did not flow down the hill. Peat sorb was spread on the oil to contain it and the polishing rod bushing was fixed. J Hawk was called and arrived on location with a single axle dump truck and an excavator. Approximately 7 tons of oil contaminated soil was removed from location and lime was spread over the remaining soil to neutralize any residual soil. The tank battery contained identification and was surrounded by a dike. The tank battery contained two (2) 210bbl production tanks, two (2) vertical separators and a poly brine tank. The poly brine tank appears to have been overflowed as a large area of oil contaminated soil is present inside the dike. The contaminated soil from the wellhead will be disposed of today and a crew will be on location at 8am 9/9/2015 to clean up and dispose of the oil contaminated soil from the dike. A follow up inspection will be conducted to ensure the spill has been completely remediated.
1441978352 2015-09-10 Production Wells No Violations I was on location to follow up to complaint #5113. An anonymous phone call was received September 7, 2015 at 4:06 pm to the Uniontown Office. The caller stated near 3750 Farber Road, Magnolia, Ohio 44643 there was a well that was leaking oil from the pumping unit and that the oil was going down a hill. The spill at the wellhead has been cleaned up and a dike has been installed around the wellhead. The oil soaked soil in the tank battery has been excavated and disposed of. Clean soil and lime to neutralize any residual product has been placed in the tank battery and all fittings have been rebuilt or tightened to prevent future leaks. The identification on the tank battery has been updated and no more spills are present. At this time there are no violations and the complaint can be closed.
1471605056 2016-08-18 Production Wells No Violations I conducted an inspection of this well per the request of Kyle Tharp (ODNR Oil & Gas Hydrogeologist) to gather field information so a determination could be made about complaint #5388 which was received on 8-03-2016 at 8:22AM. My initial inspection found the well head was buried so I contacted Tim McConanha (Enervest Superintendent) and advised him the wellhead would need to be exposed so the annulus pressure could be check. Once the wellhead was exposed Charles Youngen (Envervest Foreman) contacted me. My inspection of the wellhead found it was an open hanger completion. The RBDMS record show the surface casing was set to a depth of 940 feet but after checking the permit file some of the records are showing depth of 980 feet. The well is equipped with a pumpjack with an electric motor, it has reported production for 2015 and it is fenced in. It shares a tank battery with well # 151-24918 which is also is a well which was checked for the complaint. There are no visual integrity issues noted with either the well or tank battery at this time.
