Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1398367621 2014-04-24 Production Wells Violations Noted During my inspection I found a stable lease road leading to a pump jack well with proper I.D. signage. The tank battery for this well contains three 210bbl tanks with posted I.D. signage. A mechanical separator is present outside of the SPCC dike.Within the dike, there are two areas of oil staining around one of the older 210 bbl production tanks from apparent leaks in the tank. There also may be a leak on the side of the production tank. In addition to the stained areas, there is a large amount of trash lying around and within the diked area. I talked with pumper Frank of Downtown Petroleum (330-705-9869) and he stated that he is going to talk to Brian Levengood about removing the leaking 210bbl tank. He also stated that he will be cleaning up the tank battery. The spill in the tank battery and the debris in the dike must be cleaned up. The leaking 210bbl tank must either be removed from the dike or fixed to prevent it from leaking oil. I will be following up in 1 week to confirm that the work is bring done.
1652411829 2014-05-07 Production Wells No Violations I was on location to follow up to a compliance notice issued on 4/25/2014 for failure to keep dike or pit free of brine or other wastes. At this time the dike has been cleaned up and there is no longer any free brine and oil inside the dike. The I.D. signage is properly posted and some new rock material has been placed inside the dike were the contaminated soil was removed. This compliance notice can be passed and there are no violations at this time.
