0122631783 2005-09-20 Production Wells No Violations N.O.V. has been RESOLVED.
0204081528 2004-09-08 Production Wells No Violations NO VIOLATIONS at the tank battery.
0276227346 2004-01-07 Production Wells No Violations NO VIOLATIONS at TB.
0499805372 2015-09-23 Production Wells No Violations I was on location to witness the cleanup of an oil spill that occurred into the wetland from the flowline. The stream in the wetland has been boomed and isolated from allowing any additional oil from moving down stream. Sutton Pump crews were padding up oil and pushing oil on the surface with a leaf blower to a sump dug by a backhoe. The sump and concentrated oil is being vacuumed up by a truck. Cleanup and tank battery workover will continue tomorrow 9/24/15 until the oil has been cleaned up.
0684207385 2019-01-09 Production Wells No Violations My inspection found the well is equipped with a pumpjack. The well has reported production for 2017. The tank battery consists of two steel production tanks, two vertical separators and a meter. The well is legibly identified. The tank battery is shared with wells #151-22163 & 22164. The dike is shared with the tank battery for well #151-22258. No compliance issues are cited at this time.
0812276892 2015-09-28 Production Wells No Violations I was on location to witness the cleanup of the tank battery for this well. Four (4) 210bbl production tanks have been steamed and removed from location. The two (2) unused heater treaters and a separator were being steamed and removed from location by Sutton Supply today. The last heavy pockets of oil in the wetlands from the spill have been padded up and vacuumed from the water surface. A light rain from the previous night has freed oil from the vegetation and has and is currently mostly broken down by the exposure to the environment. The oil is isolated from moving downstream and will be removed as it accumulates.
0907120171 2015-09-24 Production Wells No Violations I was on location to witness the cleanup of the tank battery for this well. Five (5) 210bbl production tanks, two (2) unused heater treaters and a separator are being steamed and removed from location by Sutton Supply. The equipment is currently not in use and are leaking. The last heavy pockets of oil in the wetlands from the spill have been padded up and vacuumed from the water surface. The cleanup of the well and the tank battery workover will continue into next week.
1094776406 2015-09-21 Production Wells No Violations I was on location to oversee the cleanup of an oil spill that occurred on the flowline for this well. I was contacted at 9:30am by Steve Spillman of Enervest reporting the spill of approximately 3bbls of crude oil from the flowline. The spill has entered a wetland and a tributary of Little Sandy Creek. Bart Ray of the Ohio EPA was contacted and on location. The creek was isolated with a hard boom at an outflow and a sump was dug in the wetland. Equipment could not be used in the spill area due to very wet conditions. Sutton Supply crews were used for cleanup. The area was flushed with a load of freshwater and the oil and water vacuumed up with a J Hawk truck. The leak in the line has been fixed and the flowline pressure tested to ensure there are no more leaks present. Additional flushing and cleanup will occur tomorrow when daylight is present.
1442936773 2015-09-22 Production Wells No Violations I was on location to witness the cleanup an oil spill from the flowline of this well into a wetland. The Sutton Supply cleanup crew have trenched and flushed the wetland into a central sump area. A leaf blower and shovels have been used to move the oil into an area made accessible for a JHawk truck to vacuum oil from the surface. The flushing and concentrating of oil will continue and oil not able to be cleaned up in the trench will be padded up by hand.
1443641340 2015-09-30 Production Wells No Violations I was on location to witness the cleanup of the tank battery for this well. Four (4) 210bbl production tanks have been steamed and removed from location. The two (2) unused heater treaters and a separator were steamed and removed from location by Sutton Supply. The last heavy pockets of oil in the wetlands from the spill have been padded up and vacuumed from the water surface. A heavy rain from the previous night has freed oil from the vegetation and has pooled it in the dug trenches. The oil has been isolated in the ditch and the stream. Oil will be removed as it accumulates. The free oil currently present has been largely broken down by exposure to the environment. The booms and pads will be left in for the week and removed on 10/5/2015 when it is confirmed no free oil is present.
1570021221 2005-04-27 Production Wells Violations Noted Clean up the stained soil from around the wellhead.
1697353068 2015-10-13 Production Wells No Violations I was on location to check the status of the cleanup from an oil spill that occurred at this well on 9/21/2015. A heavy rain from the previous night has freed oil from the vegetation and has pooled it in the dug trenches. The last heavy pockets of oil in the wetlands from the spill have been padded up and vacuumed from the water surface. The oil has been isolated in the ditch and the stream. The free oil that was present has been broken down by exposure to the environment. The booms and pads in the stream will be removed as no oil is currently moving through the wetlands to the waterway. Identification has not been updated at the tank battery. I have called Enervest requiring the proper identification for all wells pumping into the tank battery to be posted. I will follow up to ensure the well and tank battery are brought into compliance.
