0256949080 2017-04-20 Production Wells No Violations I conducted a Compliance Notice follow-up inspection on this well. The stuffing box is repaired and the well is not causing pollution. The contamination with in the dike has been cleaned up. The well is not legibly identified. The well needs to be identified at the well and at the tank battery since it is shared with well # 151-22146 (Pickens #1). I called and left a message with Bill Arnholt and Spoke with Corey. I advised Corey the well and tank battery still needed signs at both locations in order for the Compliance notice to be passed. Corey advised me he would get the pumper to legibly identify the well.
0609187008 2017-05-03 Production Wells No Violations I conducted a Compliance notice follow-up inspection on this well. A previous compliance inspection was done on 4/20/2017 found the well was not causing pollution but was not legibly identified. The well is legibly identified. The Compliance Notice is passsed.
1454507041 2016-01-25 Production Wells Violations Noted I was on location to conduct a status check on this well. The wellhead has a pump jack on it and the valves on the wellhead are open and producing. The stuffing box was covered in paraffin and oil and oil staining was on the ground around the wellhead. The tank battery consisted of one, 210 BBL production tank, a mechanical separator and a drip tank. The 210 BBL production tank was leaking at the tank valve and staining was observed below the valve and under the tank. The flow line that connects to the mechanical separator was also leaking at one of the connections. The signage at the wellhead needs to be updated to reflect the current information. The lease access was stable. The following needs completed by:

1) Clean-up paraffin and oil around the wellhead and stuffing box (2-17-16) 2) Repair or replace the tank valve connection on flow line on the 210 BBL production tank and clean-up the stained area beneath the valve (2-17-16) 3) Repair the flow line connection where it is leaking on the mechanical separator (2-17-16) 4) Update the signage at the wellhead (2-10-16)
