0160362763 2017-12-18 Production Wells No Violations I did a status check inspection on this well to follow-up on a flow-line leak clean-u and ground restoration. The clean-up and ground restoration are partially done. I call Ron Hammen (Lakeside) contractor for the clean-up and remediation. He advised me he had missed the area immediately south of the line leak do to snow cover and would restore the contaminated area as soon as ground condition permit. The ground conditions where soft the day of the inspection. I called and spoke with the landowner.
1063249478 2017-12-11 Production Wells Violations Noted I did a status check inspection on this well after receiving a landowner inquiry about a line leak on his property. The leak is located at (40.69255, -81.29952) and the affect area is ~ 75 ft x 36 ft. There is peat-sorb material on the leak area. I call the Duty Line and gave the information to Vickie Bunting (ODNR Oil & Gas). The well is equipped with a pumpjack and it is shut-in. I received a phone call from Aaron Escott (Pumper Petrox) on 12-12-2017 he advised me the flowline leak is from well # 151-22306 (Sickafoose #5) which is also shut-in. Ron Hammon (Lakeside) called on 12-12-2017 and advised me Lakeside would be doing the clean-up and land remediation for the leak.
1311417215 2018-01-10 Production Wells No Violations I did a status check inspection on this well. The well is equipped with a pumpjack. The tank battery consists of three production tanks, a vertical separator and a meter. The tank battery is shared with wells #151-22041, 22097, 22306, 22432 and 22662. The well is legibly identified. No compliance issues are cited at this time.
1515597366 2018-01-10 Production Wells No Violations I did a compliance notice follow-up inspection for this well. I received a phone call from Ron Hammen (Lakeside) clean-up contactor on 01-09-2018 advising me clean-up operation have been completed on the portion which was missed do to snow cover. The site is graded to approximate original contour, seeded and mulched. I called and left a message for Tommy Sanscic (Landowner). The violation is passed.
