0866736588 2018-08-08 Production Wells Violations Noted I did a status check inspection for this well. The well is equipped with a pumpjack. The tank battery consists of a steel production tank and a vertical separator. The tank battery is shared with wells #151-22116 and 22118. The tank battery is overgrown with vegetation. There is no legible identification at the well. I dialed the emergency contact number (614-457-1004) listed on the tank battery sign and got an automated reply stating it was no longer a working number. I called and spoke with Corey McCudden (Northwood Energy). He advised me he would get the signage issues corrected and vegetation cut back at the tank battery.
1549300143 2019-02-04 Production Wells Violations Noted My inspection found the well has not been legibly identified at the well as requested by the compliance notice issued on 08-08-2019. The stuffing box is leaking oil and there is a contaminated area ~ 1ft. x 4 ft. around the wellhead. The tank battery consists of a steel production tank, a vertical separator. There is a domestic gas connection at the tank battery. The tank battery is shared with wells 151-22116 & 22118. The tank battery has legible identification and the well does not. I phoned and left messages with Corey McCudden (Northwood) & Bill Arnholt (Norhtwood), I advised them a Compliance Notice is being issued for pollution and contamination. Ensure the well is legibly identified at the well and tank battery - per OAC 1501:9-9-05(A)(10). Also, clean-up the oil contamination and repair the stuffing box as needed to prevent the escape of oil and gas - per AOC 1501:9-9-5(A)(1).
