0707384147 2018-09-20 Production Wells No Violations My inspection found the booms are still in place and collecting oil. The flowline has not been repaired. I called Rick Huffman (Eric Petroleum). He advised me the new line in to be buried and he would notify me when it is completed.
0887862399 2018-08-13 Production Wells No Violations I inspected two dam locations and oil is still collecting at both locations. I walked to the flowline break and the oil entry point into the stream. I met with Fred Romeo (Emergency Response Oil & Gas) on location. I called Rick Huffman (Eric Petroleum), he advised me he had been on location prior to my arrival and was aware of the oil at the dams. The monitoring of the dams will continue until the Ohio EPA allows them to be removed. I will do a follow-up inspection on a later date.
1095692319 2004-09-20 Production Wells Violations Noted 9-20-2004 Received a call from Kurt Kollar (OEPA) regarding a flow line leak that was reported to the Osnaburg Twp. Fire Dept. Inspected the site. Met with Kathy Huffman (Eric Petroleum) and Bob Ferney (330-353-6148) of the Osnaburg Fire Dept. Clean up will begin today. Remove free oil and oil stained soil. Place an I.D. at the WH. Rick Huffman (Eric Petroleum) will land farm the contaminated soil on diked plastic at the release area prior to disposal at a landfill.
1470644256 2006-02-23 Production Wells No Violations N.O.V. has been resolved. Approximately 30 tons of remediated soil has been hauled off the location. The area will be seeded in April. A new I.D. sign will be placed at the wellhead next week.
1533564872 2018-08-01 Production Wells No Violations I received a phone call from Scott King (ODNR - Oil & Gas) at 8:17pm on 08-01-2018, he advised me there had been a spill in a creek near Stark Ceramics (SC). I contacted the Vikki Bunting (Duty Line), she gave me well #151-21122 (Stark Ceramics #2) as a starting point. I arrived at well #21122 no personnel were there. I move to Orchardview Dr. and met with Kurt Kollar (OEPA), Rick Huffman (Eric Petroleum) and Ohio Wildlife Officer Jesse Janosik. The left was from a broken flowline used by wells 151-21883 (SC #14) and 21824 (SC #8). I spoke with Rick Huffman. He advised me the #8 has been shut-in and it was the SC #14 which had been producing. Booms where set in the creek at varied location downstream from the spill location. I will do an inspection for the well on 08-02-18 and a follow-up on the clean-up.
1640797598 2018-12-04 Production Wells No Violations My inspection found the rigid boom has been removed and the well is no longer causing pollution. The compliance notice is passed.
1850793162 2018-08-02 Production Wells Violations Noted I did a status check inspection for this well and an inspection to check clean-up progression for a broken flowline. The well is equipped with a pumpjack and it is shut-in. The tank battery consists of a steel production tank, two vertical separators and an inground brine vault. The flowline is severed at +/-9 (40.78276, -81.29675) and it appears to have been caused by an ATV. There are ATV tracks throughout the property. Rick and Vicky Huffman (Eric Petroleum), Kurt Kollar (OEPA), Fred Romeo (ODNR-Oil & Gas) and Sweet Meadows Farm Drainage are on location for the spill clean-up. Several dams and booms have been placed along the affected un-named stream to aid in the clean-up. I phoned Rick Huffman (Production Superintendent - Eric Petroleum) on 8/6/2018 and advised him a violation is going to be issued with an abatement date of 8/20/2018.
1984302878 2004-09-24 Production Wells No Violations The oil contaminated soil has been cleaned up and placed on plastic at the site. The soil will be treated and then taken to a landfill for disposal.
