0017001426 | 2012-11-20 | UrbanProduction Wells | No Violations | I received a call from Kyle Tharp on November 20, 2012 regarding a water well concern from David McKinney at 4262 Mapleton Street, Canton Township. I inspected the Mayes J 1 well (34151218640000) on November 21, 2012. The operator for this well is Enervest Operating LLC. 2011 production for this well includes 188 bbls of oil, 16 bbls of brine, and zero gas. My inspection found the lease road was not graveled to the wellhead and showed no signs of erosion. The wellhead includes a gas powered pumping unit. The pumping unit was not running at the time of my inspection. The pressure gauges attached to the tubing recorded 0 psi and the 4.5 inch casing recorded 0 psi. Valves are open from the tubing to the tank battery. There is minor oil staining surrounding the wellhead. A hissing sound can be heard from the wellhead and gas bubbles can be seen escaping between the tubing and the 4.5 inch casing. Bull plugs on the tubing and 4.5 inch casing are missing from the wellhead assembly. The tank battery consists of one 210 bbl stock tank and one concrete brine tank. There is no separator at this site. The 210 bbl stock tank has the proper operator sign attached. The dike is in good condition with containment capacity. All spacing requirements have been met. According to Troy Wells at Enervest Operating LLC, a service rig was on site in April 2012 to replace rods.

1453475454 | 2016-01-15 | Production Wells | No Violations | I was on location with Regional Emergency Manager Terry Whitehair to follow up to a flowline leak that occurred at 9:30 on 1/14/2016 and was reported at 12:10 1/14/2016 by the operator. The flowline was corroded and 1.5bbls of oil spilled from the flowline onto the ground. The ground was frozen and snow was present when a J Hawk water truck was on location to vacuum up the oil. All of the oil was vacuumed up and no soil excavation was needed. The flowline was clamped and pressure tested without failure. The operator spread lime on the location to neutralize any free residual oil. The well is an gas driven pumpjack well. The tank battery is identified, surrounded by a dike and contains a 210bbl production tank and a concrete brine vault. At this time the spill has been remediated. |
