0846727856 2014-07-11 Production Wells No Violations I was on location to check on cleanup for a spill that occurred on this well. The cleanup for this well has been completed by Sunroof Environmental and has been approved by OEPA. The spill has been cleaned up and the well and tank battery has been made to no longer spill oil or brine. A new I.D. sign has been installed on the tank battery, a dike has been built up around the tank battery and locks have been installed on the tank battery and the wellhead. At this time all issues out of compliance for this well have been passed and the compliance notice for this well has been closed..
1108171955 2014-06-05 Production Wells Violations Noted I was on location to check on cleanup for a spill that occurred on this well. Currently Sunpro Environmental services are on location. An additional check damn has been installed to prevent the flow of oil to the creek from the field tile and several oil seeps have been found and are contained below grade. Filter socks and adsorbent pads are distributed throughout the creek to adsorb free oil. Labor crews have been brought onto location tomorrow to enter the woods and start the vegetation and oil clean up inside the woods. At this time the oil has been isolated from entering the creek and contained in drainage ditches. A ditch has been dug so that water from the field bypasses the contaminated soil. They expect cleanup to continue into next week.
1401730009 2014-06-02 Production Wells Violations Noted I was called by Wade Balser (OEPA 216-789-9281) regarding an oil spill in a Nimishillen Creek tributary. The spill appears to be over a week old with residual oil left in the creek. There is a grass kill on the high water line of the creek. We identified the source of the oil to be well Schmuck #3. The well is a swab well with a 2 ball valve on the 4.5 production casing. It is used as a domestic well to heat an office on the farm. A flow line from the casing runs to an unmarked 100bll production tank. The area around the well and the tank have been recently dug up. There is a steel field tile pipe dumping into the creek and has an oil sheen on it. We met with the operator on location and he stated 2 weeks ago the cows opened the valve on the oil production tank causing an unknown amount of oil to spill into the creek. He thought that he had cleaned it all up with a track hoe. He has recently removed the handle to the valve so that it will not spill anymore. A contractor is on route to the location to clean up the spill. The spilled oil in the creek must be cleaned up and the oil in the field tile drain must be stopped. The well needs proper I.D. signage for identification. A dike must be built around the production tank to contain any spills and the tank battery and well needs to have bull plugs and locks installed to prevent future spills.

Currently Wade Balser of the OEPA and Sunpro Environmental services are on location. A check damn has been installed to prevent the flow of oil through the creek.
