0256256235 2017-08-23 Production Wells No Violations I did a status check inspection on this well and Luke Smith (Survey Tech. Oil & gas) was in accompaniment. My previous inspection found the wellhead had been buried by the coal company during the construction of a wetland area on Coal Permit D-2346 (Mapleton). The condition of the well head is unknown at this time. A metal detector was used to locate the magnetic signature of the casing and the well location was surveyed.
0313999491 2017-08-14 Production Wells No Violations Barry Alexin (Rosebud Mining) contacted ONDRs Division of Oil & Gas and advised that Rosebud Mining hit the wellhead on this well while developing a wetland. The landowner had contacted Rosebud Mining about the field crew hitting and burying the well. My inspection found the wellhead below ground level with a perforated PFC pipe with a plugged black cap extending above ground. I spoke with Barry Alexin and will meet with him on Tuesday 8/15/2017 at 8:30am to affirm this is the well which was hit. Aaron Lantz (ODNR Oil & Gas) was notified about the well plotting incorrectly on the well locator.
1130659941 2018-11-21 Production Wells No Violations My inspection found Hagen well service on location getting ready to trip in tubing to place a bottom plug. I received a phone call from Brian Levengood (Dover Atwwod Corp.), he advised me the tubing landed at 3750 Ft.
1536589291 2018-09-07 Production Wells Violations Noted With Tom Hill (Regional Supervisor - ODNR, Division of Oil & Gas), My inspection found the well is still buried. The well was struck and buried by equipment the Rosebud Mining Company was using in 2017 during the construction of a wetland on mine permit D-2346 (Mapleton). The Rosebud Mining Company has not restored the ground to get the well to its previous above ground level. The construction and the condition of the well are unknown since the well has been buried to an unknown depth. The well is idle and the well last produced in 1993 for 50 barrels of oil. There are no annual production reports submitted from 1995 through 2005. The annual production reports from 2006 through 2017 show zero production. There is no tank battery associated with this well. The wells location was surveyed on 8-23-2017 using the magnetic signature of the casing. An above ground plastic PVC pipe shows the location This well presently is incapable of production. Consequently, this well shall be plugged in accordance with Ohio Revised Code section 1509.12(B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
